1. Approve form and content of the Register of subagents, salaried employees which function is mediation, and broker assistants to the insurance and/or reinsurance intermediary, according to appendix 1.

2. Approve form and content of the Report on the main activities of the insurance and/or reinsurance intermediary, according to appendix 2.

3. Approve form and content of the Activities report of the insurance and/or reinsurance intermediary by means of salaried employees, broker assistants and/or subagents, according to appendix 3.

4. Approve form and content of the Report on debts of the insurance and/or reinsurance intermediary to insurers, according to appendix No. 4.

Procedure for filling of the activities report of the insurance and/or reinsurance intermediary by means of salaried employees, broker assistants and/or subagents

In column 2 are specified added gross - awards, including by means of salaried employees, broker assistants to physical persons, broker assistants to legal entities, subagents, and their total amount is equal to the amount registered in column 5 of the report on the main activities of the insurance and/or reinsurance intermediary for the same accounting period;

3. Insurance and/or reinsurance intermediaries will specify data in the Report as follows:
