Application process on production of technical means of rehabilitation by the individual order

1. The application process on production of technical means of rehabilitation determines procedure of registration and issue of the technical tool of rehabilitation for the individual order by the individual order.

2. The questionnaire for production of the technical tool of rehabilitation by the individual order (further - the questionnaire) is filled in with medico-social commission of experts or the medical and advisory commission according to form (appendix) based on the address of person having the right to ensuring rehabilitation with the technical tool (further - person) that according to the conclusion of medico-social commission of experts or the medical and advisory commission cannot use technical means of rehabilitation of serial production (further - the conclusion). 3. Local authorities of work and social protection of the population monthly to the 15th generalize questionnaires and give the relevant information to the Ministry of Labour and Social Policy of the Autonomous Republic of Crimea, head departments of work and social protection of the population regional, to Head department of social protection of the population Kiev and to Management of work and social protection of the population Sevastopol the city public administrations (further - Head departments of work and social protection of the population).

4. Head departments of work and social protection of the population monthly to the 20th submit the generalized applications for production of the technical tool of rehabilitation by the individual order to Social Security Fund of disabled people.

5. Social Security Fund of disabled people in week time considers the request and approves production of the technical tool of rehabilitation by the individual order with the company contractor of the state order, according to the Procedure for ensuring separate categories of the population with technical and other means of rehabilitation and forming of the corresponding state order, list of such means approved by the resolution of the Cabinet of Ministers of Ukraine of 29.11.2006 N 1652.

6. The company contractor independently determines necessary measures for production of the technical tool of rehabilitation by the individual order.

7. Payment of the technical means of rehabilitation manufactured and delivered by the companies contractors for the individual order is carried out by Social Security Fund of disabled people according to the contracts signed with them, registers for the issued technical means of rehabilitation by the individual order to TsBI, delivery-acceptance certificates of works (provision of services) by local authorities of work and social protection of the population, calculations and calculations on individual production of products.

8. Local authorities of work and social protection of the population in case of issue of the technical tool of rehabilitation by the individual order bring information in TsBI.

9. In case of factual determination of availability of the similar product made in the conditions of serial production with the parameters necessary for person, and inexpediency of production of the technical tool of rehabilitation by the individual order Social Security Fund of disabled people reports about it in writing to head departments of work and social protection of the population, and person is provided with means of rehabilitation of serial production.

10. Information on refusal in production of the technical tool of rehabilitation by the individual order by local authorities of work and social protection of the population is noted in TsBI.
