About approval of form of the certificate on state registration of non-profit organization

1. Approve the enclosed form of the certificate on state registration of non-profit organization.

2. To department for non-profit organizations (Milushkin of Page Yu.) and to territorial authorities of the Ministry of Justice of the Russian Federation to perform issue of the certificate on state registration of non-profit organization:

in case of state registration of non-profit organization during creation;

in case of change of the data containing in the certificate in connection with entering into the Unified State Register of Legal Entities of the record attracting such changes;

when losing of the certificate by non-profit organization with submission of the acts, references and other documents confirming the fact of loss of the certificate;

in case of damage of the certificate attracting impossibility of its use, in case of submission of the original of the damaged certificate;

instead of the certificate granted to the publication of the order in case of the appeal of non-profit organization about state registration or entering into the Unified State Register of Legal Entities of changes;

with inclusion in again (repeatedly) granted certificate of data on earlier granted certificate;

with inclusion of data on the executive body which made the decision on state registration of non-profit organization in the certificates granted to the non-profit organizations created after entry into force of the Federal Law of August 8, 2001 by N129-FZ "About State Registration of Legal Entities and Individual Entrepreneurs" (The Russian Federation Code, 2001, N 33, Art. 3431; 2003, N 26, Art. 2565; Art. 3. To department of administration (Rumyantsev S.A.) organize production of forms of the certificate on state registration of non-profit organization.

4. Declare invalid the order of the Ministry of Justice of the Russian Federation of August 1, 2008 of N161 "About Approval of Form of the Certificate on State Registration of Non-profit Organization" (it is registered by the Ministry of Justice of the Russian Federation on August 12, 2008, registration N 12103).

5. This order becomes effective after thirty days from the date of publication.
