About approval of industry regulations concerning fire safety of "The fire safety regulation in case of operation of trunk oil pipelines of Ukraine"

In pursuance of Section 1 of the State program of ensuring the fire safety for 1995-2000 approved by the resolution of the Cabinet of Ministers of Ukraine of April 3, 1995 N 238, and Item 3.2.1 of the Program of development of regulations on labor protection of Gosneftegazprom for 1996 and also for the purpose of prevention of the fires at the companies of trunk oil pipelines, - PRIKAZYVAYU:

1. Approve the Fire safety regulations in case of operation of trunk oil pipelines of Ukraine developed by JSC Institute of Oil Transportation with attraction of UKRNIIPB of the Ministry of Internal Affairs of Ukraine and introduced by the company of the Dnieper trunk oil pipelines which are applied.

2. Enact Fire safety regulations in case of operation of trunk oil pipelines of Ukraine in the territory of Ukraine since January 1, 1998.

3. To consider the fire safety regulations in case of operation of trunk oil pipelines approved by Minnefteprom of the USSR of 26.10.1981 since January 1, 1998 such which are not applied in the territory of Ukraine.

4. Submit to JSC Institute of Oil Transportation these Rules to UGPO Ministry of Internal Affairs of Ukraine for inclusion in the state register, according to the procedure, the provided Regulations on procedure for development, approval, viewing, cancellation and registration of regulations concerning fire safety approved by UGPO Ministry of Internal Affairs of Ukraine.

5. The edition and distribution of these Rules is entrusted to the company of the Dnieper trunk oil pipelines with involvement of JSC Institute of Oil Transportation.

Fire safety regulations in case of operation of trunk oil pipelines of Ukraine

1. Scope

1.1. Fire safety regulations in case of operation of trunk oil pipelines of Ukraine (further - Rules) establish the main requirements of fire safety in case of operation of trunk oil pipelines of Ukraine for operating plants of trunk oil pipelines.

Rules extend to the Companies of trunk oil pipelines which are operated, under construction, reconstructed, technically are re-equipped and extend.

Treat the Companies of trunk oil pipelines:

the main, relay oil pumping stations, bulk and terminal stations (terminals) of trunk oil pipelines with the reservoir park or without it, with complex of buildings, constructions and engineering communications for ensuring operational activity on acceptance, storage and transportation or oil sale;

linear part of trunk oil pipelines (actually the pipeline) with branches and loopings, shutoff valves, nodes of start-up and acceptance of cleaning devices, life support systems, telemechanization and scheduling.

1.2. These Rules are obligatory for application by all companies, organizations, organizations (irrespective of types of their activities and patterns of ownership) and their workers.

2. Normative references

2.1. Terms and determinations which are used in these Rules, GSTU 2272-93, GSTU 2273-93, state standard specifications 12.1.004-91, state standard specifications 12.1.044-89, state standard specifications 19433-88, ONTP 24-86, Construction Norms and Regulations 2.05.06-85, BCH B.2.2-58.1-94, BCH B.2.2-58.2-94 answer the Law of Ukraine "About pipeline transport".

The list of regulating documents and reducing on them which are used are given in the information supplement N 14.

References to requirements of regulating documents which should be considered on tightly specialized questions are provided in appropriate sections and subsections of these Rules.

3. General provisions

3.1. Fire safety shall be ensured by carrying out the organizational, technical and other measures directed to prevention of the fires, safety of people, decrease in possible losses and reduction of negative ecological effects in case of their origin, creation of conditions for urgent challenge of fire divisions and successful clearing of the fire.

3.2. In the territory, in buildings, constructions and premises of the companies and other objects of trunk oil pipelines (daleepredpriyatiya) the fire prevention regime corresponding to their fire risk * shall be set, including shall be determined:

- possibility (places) of smoking, use of naked flame, household heating appliances (on subsection 6. 2);

- requirements for content of places where smoking, use of naked flame, household heating appliances is allowed;

- procedure for carrying out temporary hot work (including welding) - according to Section 10;

- storage locations and admissible amount of raw materials, semifinished products and finished goods which can be at the same time in production placements and on the territories (in storage locations) - according to Section 9;

- procedure for cleaning of combustible dust and waste, the poured oils or oil products, storages of the oiled overalls and rags, cleanings of air ducts of ventilating systems of combustible deposits - according to Section 5;

- inspection routine and closings of rooms after completion of work;

- procedure for passing by officials of training and examination concerning fire safety, and also carrying out with workers of fire-proof instructing and classes in fire technically minimum with appointment responsible for their carrying out (according to the Section 4, to annexes of NN 1, 2, 3 and to recommended appendix N 4);

- procedure for the organization of operation and servicing of available technical means of fire protection (fire-proof water supply system, pumping points, installations of the fire alarm, automatic fire extinguishing, smoke removal, fire extinguishers, etc.) (according to the Section 8);

- procedure for carrying out scheduled preventive maintenance and surveys of electric equipment, heating, ventilating, processing and other engineering equipment;

- rules of journey and parking of vehicles;

- obligations and actions of workers in case of fire identification (the Section

11);

- procedure for collection of members of voluntary fire team and ranking officers in case of the fire, their challenge at night, days off and holidays.

The procedure for establishment of fire prevention regime is given in subsection 4.1 of these Rules.

- ensuring fire safety of the Companies of trunk oil pipelines and other objects of trunk oil pipelines is assigned to their heads and the faces authorized by heads;

- ensuring fire safety is component of productive activity of heads and employees of the companies that shall be displayed in employment contracts (contracts) and charters of the companies.

3.4. Categories of rooms and buildings on fire and explosion and fire risk are accepted depending on engineering procedures which are placed in rooms, on the special qualifier approved by Gosneftegazprom or the annex of N 3 VSN B.2.2-58.1-94 taking into account subsection 4.3.

3.5. Introduction (introductory) fire-proof instructing there shall undergo all persons who are employed, according to requirements of subsection 4.5 of these Rules.

3.6. At the Companies of trunk oil pipelines and other objects of trunk oil pipelines voluntary fire teams (teams) can be created.

The procedure for creation of fire-proof forming is given in subsection 4.6 of these Rules.

3.7. The machines acquired abroad, mechanisms, the equipment, vehicles, processing equipment (including fire-fighting and electric equipment in explosion-proof execution) are put into operation only on condition of compliance to their regulations on fire safety which are effective in Ukraine.

4. Organizational measures concerning ensuring fire safety

4.1. Establishment of fire prevention regime

4.1.1. Fire prevention regime is set by the order of the director or the instruction approved by it.

4.1.2. All employees of the company shall be informed of requirements concerning providing fire prevention regime on instructing, when passing fire and technical minimum, and also on occupations in system of inservice training.

4.1.3. Statements of basic provisions from the order (instruction) should be hung out on foregrounds.

4.1.4. The director shall accept (within the rights granted to him) adequate measures of reaction on the facts of violation or failure to carry out by officials and other employees of the company of fire prevention regime.

Works managers who are carried out by contract organizations shall provide observance by employees of contract organization of the fire prevention regime set at the company.

4.2. Determination of obligations of officials and workers concerning ensuring fire safety, appointment responsible for fire safety

4.2.1. The director (through the corresponding workers) shall:

- develop complex measures concerning ensuring fire safety, implement achievement of science and technology, positive experience of prevention of the fires;

- according to regulations on fire safety to develop and approve provisions, instructions and other regulations which are effective in borders of the company, to exercise constant control behind their observance;

- establish in the territory, in buildings, constructions and rooms fire prevention regime and to provide its observance by all who works;

- provide observance of fire-proof requirements of standards, regulations, rules, and also fulfillment of requirements of bodies of the state fire supervision;

- organize training of workers in fire safety regulations and promotion of measures concerning their accomplishment;

- in case of absence in regulations of the requirements necessary for ensuring fire safety to take adequate measures, approving them with bodies of the state fire supervision;

- provide in necessary quantity and contain in good repair means of fire protection and communication, the fire fighting equipment, the equipment and stock, not to allow their use not to destination;

- create in case of requirement in accordance with the established procedure divisions of fire protection and material and technical resources, necessary for their functioning;

- create voluntary fire teams (teams), the fire engineering commissions and organize their activities according to requirements of acts of the legislation;

- provide upon the demand of the State fire protection the information and documents on condition of fire safety of objects and products which is made by them;

- perform actions for implementation of automatic means of identification and clearing of the fires and use for this purpose of production automatic equipment;

- to timely inform fire protection on defect of the fire fighting equipment, fire protection systems, water supply, and also on closing of roads and drives in the territory;

- make special investigation of cases of the fires.

The director shall determine also obligations of workers concerning ensuring fire safety, to appoint certain buildings, constructions, rooms, sites and other objects, the processing and engineering equipment, responsible for fire safety, for content and operation of technical means of fire protection.

4.2.2. Obligations of workers concerning ensuring fire safety, content and operation of technical means of fire protection shall be displayed in the relevant official documents (functional obligations, job descriptions, provisions, etc.).

4.2.3. Obligations of the Parties concerning ensuring fire safety of leased property shall be determined in the lease agreement.

4.2.4. Responsibility for fire safety of workshops, laboratories, departments, masterful, reservoir parks, pumping points on transfer of oil and other production sites of the company (further - structural divisions) is assigned to their heads, and during lack of the last - to workers who carry out their obligations.

Responsibility for observance of the established fire-proof actions on each workplace is conferred on the worker who services this site of work that shall be displayed in the relevant official documents (lists of functional obligations, instructions, provisions and so forth).

4.2.5. In each room (pumping, laboratories, ventilating installation, masterful, etc.) the plate with indication of surname, name, middle name and position of the worker responsible for fire safety shall be in a visible place hung out.

4.2.6. The heads of structural divisions, other workers appointed responsible for fire safety shall:

- provide observance by workers established at the company or in structural division of fire prevention regime, fire safety regulations and instructions about actions of fire safety;

- know characteristics of fire risk of buildings, constructions, engineering procedures, processing and production equipment which stay in the address are used, made and stored in production of substances and materials;

- carry out periodic (at least once a month) surveys of the territories entrusted to them, buildings, rooms, constructions, production sites for the purpose of control of observance of the set fire prevention regime and to take necessary measures for elimination of the found shortcomings;

- organize control of operability of systems of heating, ventilation, electroinstallations, processing and other production equipment, to take measures for elimination of the found defects owing to which emergence of the fire is possible;

- after completion of work to provide cleaning of workplaces and rooms, shutdown of electroinstallations, except for emergency lighting, fire-proof and security installations, and also electroinstallations which under the terms of technology shall work around the clock;

- the nobility of the rule of content and use of the fire fighting equipment (including emergency firefighting equipment, automatic installations of fire extinguishing and the fire alarm), means of communication, systems of antismoke protection and the notification of people about the fire which are in division and to provide their permanent readiness for action;

- not allow to work of persons who did not undergo fire-proof instructing, daily examination or those which did not pass tests according to the program of fire and technical minimum;

- develop plans of evacuation of people and material values for case of emergence of the fire and carry out at least once a year their practical exercise according to the procedure established at the company.

4.2.7. The workers responsible for operation of processing equipment and engineering constructions shall:

- organize timely performing routine inspections and planovopredokhranitelny repairs of the equipment, installations, power supply networks, etc., and also elimination of the found violations which can lead to the fire;

- organize training and instructing of service personnel concerning ensuring fire safety in case of operation (repair) of the processing and engineering equipment;

- provide development of necessary operational documentation and control of its maintaining, to develop and perform measures concerning prevention of the fires from the processing and engineering equipment.

4.2.8. Each worker shall:

- adhere established at the company, in division, on workplace of fire prevention regime, fulfill requirements of rules and other regulations for fire safety which act on the company;

- use only operational tools, devices, the equipment, etc., to follow instructions for their operation, instructions of the heads and persons responsible for fire safety;

- carry out cleaning of workplaces from combustible waste and switch off electroreceivers (those which under the terms of technology shall not work around the clock) upon termination of work;

- be able to apply fire extinguishing means which are in division;

- in case of identification of the fire or ignition to be effective according to requirements of Section 11 of these Rules.

4.3. Determination of categories of rooms and buildings on fire and explosion and fire risk, establishment of classes of zones according to PUE

4.3.1. For all buildings and rooms of production, warehouse appointment and laboratories the category shall be determined by fire and explosion and fire risk proceeding from requirements of ONTP 24-86 and item 3.4 of Rules.

4.3.2. Determination of category of buildings and placements on fire and explosion and fire risk on blueprint stages shall be carried out by developers of engineering procedure, according to item 3.4 of these Rules.

For operating plants of category of buildings and rooms are determined by fire and explosion and fire risk by the organizations which have the corresponding specialists and permission (license) of body of the state fire supervision to the right to be engaged in such activities which is issued according to the Instruction about procedure of licensing of production of the fire fighting equipment, fire-fighting equipment, provision of services and performance of works of fire-proof appointment approved by the order of the Ministry of Internal Affairs of Ukraine of 29.03.95 of N 189, of Ukraine registered in Ministry of Justice of 17.07.1995 for N 226/762.

4.3.3. By technologists and electricians of the project organization classes of explosive and fire hazardous zones, including for external production and racking areas, open technology installations and constructions shall be established according to PUE.

4.3.4. About entrance to the building and the room plates with indication of category of fire and explosion and fire risk of the room and class of zone according to PUE shall be hung out. Classes of zones should be marked also on borders of zones in rooms and from the outside.

4.4. Development of instructions about actions of fire safety and plans of evacuation

4.4.1. At each company shall be developed:

- the all-object instruction about actions of fire safety;

- instructions for all explosion-fire-hazardous and fire-dangerous rooms of production sites, workplaces (workshops, warehouses, workshops, laboratories of reservoir parks, pumping points for pumping of oil and oil products, drain and bulk platforms and moorings, refueling Items, etc.);

- instructions for carrying out fire-dangerous work types, operation of technology installations, etc.

Instructions shall be hung out on foregrounds and be studied when carrying out fire-proof instructing, passing special training (fire and technical minimum), and also in system of inservice training.

Requirements of supervision bodies (labor protection, fire safety) can unite at the discretion of the owner in the single instruction, on condition of obligatory display of requirements of the annex in them N 1.

4.4.2. The main requirements to contents of instructions are stated in annex 1 to these Rules.

In instructions actions of fire safety shall be displayed also in case of:

- stop and cutting off of processing equipment and technology binding of oil pumping stations from trunk oil pipeline;

- the automatic omission of flow of oil bypassing oil pumping stations.

4.4.3. The all-object instruction is developed by administration of the company with the assistance of specialists of object fire protection (in case of its availability), the chief of DPD, security service of work, other interested services and divisions of the company.

The all-object instruction affirms the director or person who carries out its obligations.

4.4.4. Instructions about actions of fire safety are, etc. developed for certain rooms, production sites, workplaces, fire-dangerous work types by heads and specialists of the relevant structural divisions, services, approved with object fire protection or the chief of DPD, other interested services and officials of the company, affirm the deputy manager or the chief engineer of the company.

4.4.5. The instruction in which their obligations are determined by control of observance of fire prevention regime, survey of the territory and rooms, operations procedure in case of identification of the fire, operation of means of the fire alarm and automatic fire extinguishing shall be developed for workers of protection (watchmen, porters, persons on duty, etc.) by the director (through the corresponding workers). In it also it shall be specified which of directors needs to be called at night, in days off and holidays in case of fire. The list of officials of the company with indication of their home address, office and home phones shall be attached to the instruction. The instruction shall be hung out in locations of protection (on checkpoints, posts, etc.) and to be the protection learned by all workers.

4.4.6. In buildings and rooms which have two floors more, in case of simultaneous stay on the floor it is more than 25 workers, plans (schemes) of evacuation of people in case of fire shall be developed and hung out on foregrounds.

Need of providing with plans (schemes) of evacuation of one-storey buildings and constructions is determined by the director, in coordination with local authorities of the state fire supervision proceeding from fire risk of these buildings and constructions, the number of people who accommodate of the area, etc.

4.4.7. Plans of evacuation shall contain schematic designation of rooms of the respective floor, emergency exits and ways of movement to them, locations of fire extinguishing means and the alarm system. These plans shall have necessary explanatory texts.

4.4.8. On doors of entrance to the cellar it is necessary to hang out the plan of basements.

4.4.9. At the companies the procedure (system) for the notification of people about the fire which shall be specified in explanations to plans of evacuation (or in instructions) and with which it is necessary to acquaint all workers shall be established.

4.4.10. In case of change of the layout or functional purpose of buildings (rooms, constructions), the production technology, regular arrangement of personnel the director or structural division shall provide timely conversion of plans of evacuation and instructions.

4.5. Training in actions of fire safety, evident propaganda and signs of safety

4.5.1. Training in actions of fire safety of the trunk oil pipelines working at the companies consists from:

- instructing in questions of fire safety (further fire-proof instructing);

- training and examination of officials who carry out the obligations connected with ensuring fire safety of objects with the raised fire risk;

- special training (classes in fire technically minimum) for persons who are employed, connected with the raised fire risk.

4.5.2. Fire-proof instructing share on: introduction, primary, repeated on workplace, unplanned and target.

Introduction fire-proof instructing is carried out in specially equipped place with all workers who are employed (permanent or temporary), and also with persons who arrived to the company to business trip, on work practice (training) and shall participate directly in production process.

For carrying out introduction fire-proof instructing at the company the room equipped with necessary visual aids (posters, schemes, prototypes, etc.) which illustrate requirements of fire prevention regime in the territory of the company, the site in workshops, buildings, constructions and on workplaces, and also equipped with samples of different types of emergency firefighting equipment, fire stock and fire communication which is on object shall be allocated.

Primary fire-proof instructing is carried out by the works manager directly on workplace prior to the beginning of productive activity of the worker.

Repeated fire-proof instructing is carried out by the works manager at least once a year on workplace with all workers.

Unplanned fire-proof instructing is carried out:

- when commissioning of the new or changed regulations concerning fire safety;

- in case of changes of engineering procedure, application new, replacements or upgrades of the operating fire-dangerous equipment;

- upon the demand of the state fire supervision inspector if unsatisfactory knowledge workers of fire safety regulations is revealed, inability to be effective in case of fire and to use emergency firefighting equipment.

Target fire-proof instructing is carried out with workers before accomplishment of one-time (temporary) fire-dangerous works by them, in case of accident elimination, etc.

4.5.3. Concerning fire safety employees of the companies and their structural divisions which carry out the obligations connected with ensuring fire safety of objects with the raised fire risk shall undergo training and examination.

The marked-out workers undergo training and examination prior to accomplishment of the obligations, and afterwards - with frequency once in three years.

The procedure for the organization of their training and examination is determined by the List of positions, in case of appointment to which faces shall undergo training and examination concerning fire safety and is registered by procedure for its organization which is approved by the order of the Ministry of Internal Affairs of Ukraine of 17.11.94 of N 628, in Ministry of Justice of Ukraine of 22.12.94 for N 307/517 based on the resolution of the Cabinet of Ministers of Ukraine from

26.07.94 N 508 "About measures concerning accomplishment of the Law of Ukraine On fire safety" (the annex N 2 to Rules).

4.5.4. Special training (fire and technical minimum) there shall undergo previously (prior to independent performance of works) persons who are employed, connected with the raised fire risk.

For the purpose of increase in general knowledge of questions of fire safety once a year the workers occupied at works with the raised fire risk undergo examination of the corresponding regulations on fire safety.

4.5.5. The procedure for the organization and carrying out fire-proof instructing, special training and examination are determined by Standard regulations on special training, instructing and examination concerning fire safety at the companies by fire technically minimum, in organizations and the organizations of Ukraine, the approved order of the Ministry of Internal Affairs of Ukraine of 17.11.94 N 628 (the annex N 3 to Rules).

The approximate list of works (professions, positions) to which in case of acceptance persons shall undergo special training (fire and technical minimum) and annual examination is given in the recommended appendix N 4 to Rules.

4.5.6. The admission to work of workers who were not trained, fire-proof instructing and examination concerning fire safety is forbidden.

4.5.7. Studying of actions of fire safety at the company should be provided in system of inservice training of workers also.

For this purpose also local systems of broadcasting, evident propaganda and other means which are at the company shall be used.

4.5.8. In the territory and in premises of the companies on foregrounds it is necessary to hang out posters, stands and other evident propaganda which propagandizes requirements of fire safety regulations.

4.5.9. The territory of the company, and also the building, construction, the room shall be provided with the corresponding signs of fire safety according to GOST 12.4.026-76.

4.5.10. The locations of signs of safety, the procedure for application of explanatory texts to them is established by administration of the company in coordination with object fire protection or the chief of DPD.

Signs of safety shall be located so that they were well visible (are noticeable) and stood out surrounding them.

On the places and sites which are temporarily dangerous it is necessary to establish figurative signs of safety which are removed after need for their application disappears.

Signs which are used in night-time or in the conditions of insufficient visibility shall be noticeable - are allocated to the corresponding contrast with paint, or are coated the corresponding reflecting capability, or are allocated with artificial lighting.

The safety signs with artificial lighting established in explosion-fire-hazardous and fire-dangerous rooms (zones) or on doors of these rooms shall have the corresponding degree of protection.

4.5.11. In the territory of the company on visible (noticeable) places plates with indication of procedure for challenge of fire protection, signs of the locations of emergency firefighting equipment, the traffic pattern of transport on which it is necessary to mark arrangement of buildings, of constructions, reservoirs, hydrants, piers and coolers shall be established (need of establishment of such scheme at each company is determined with object fire protection or local authorities of the state fire supervision).

4.6. Creation of fire-proof forming

4.6.1. For the purpose of employees engagement to holding actions for prevention of the fires and the organization of their clearing at the companies and other objects of trunk oil pipelines voluntary fire teams and teams are formed.

Creation of voluntary fire teams (teams) is assigned to the director. Their activities shall be performed according to the Regulations on voluntary fire teams (teams) approved by the order of the Ministry of Internal Affairs of Ukraine of 27.09.94 of N 521, registered in Ministry of Justice of Ukraine of 13.10.94 for N 248/458 based on the resolution of the Cabinet of Ministers of Ukraine of 26.07.94 N 508.

Measures concerning stimulation and encouragement of members of voluntary fire teams (teams) (the sizes of additional vacation, cash bonuses, valuable presents, etc.) shall be provided in collective agreements.

Procedure for providing voluntary fire team (team) with overalls of members at the companies of trunk oil pipelines and regulations of accessory are given in the recommended appendix N 5 to Rules.

4.6.2. At the company with the number of 50 and more workers the solution of labor collective creates the fire and technical commissions. Based on this decision the order of the director establishes staff of the fire and technical commission and procedure for its work.

Work of the commission needs to be organized according to the Standard regulations on the fire and technical commission approved by the order of the Ministry of Internal Affairs of Ukraine of 27.09.94 N 521 (the help appendix N 6 to Rules).

4.6.3. In the device of each consolidation of the companies (association, corporation, concern, etc.) for accomplishment of the functions delegated to consolidation in the field of fire safety the service of fire safety shall be created.

Activities of service of fire safety shall be regulated by Gosneftegazprom provision which is developed according to the Standard regulations on service of fire safety approved by the order of the Ministry of Internal Affairs of Ukraine of 12.04.95 of N 220, registered in Ministry of Justice of Ukraine of 20.04.95 for N 118/654 based on the resolution of the Cabinet of Ministers of Ukraine of 03.04.95 N 238 "About approval of the State program of ensuring fire safety for 1995-2000".

5. Main requirements of fire safety to the main technology objects

5.1. General requirements to engineering procedures and the equipment

5.1.1. The course of engineering procedures shall occur according to technology documentation (production schedules, rules of the technical operation, etc.) approved in accordance with the established procedure.

5.1.2. At each company it is necessary to have data on indicators of fire and explosion and fire risk of substances and materials in accordance with GOST 12.1.044-89, which are applied in engineering procedures. It is forbidden apply in engineering procedures of substance and materials with unknown indicators of fire risk.

5.1.3. Processing equipment, held for use explosion-fire-hazardous and flammable substances and materials, shall meet the requirements of the existing regulations for labor protection and passports of the manufacturing plants.

5.1.4. Arrangement of processing equipment shall answer regulations of process design.

5.1.5. Parameters of operating mode of the processing equipment connected using flammable (LVZh) and the combustible liquids (CL) and also explosion-fire-hazardous dust, shall answer the technology documentation developed taking into account the existing regulations.

5.1.6. In processing equipment concentration of combustible gases, vapors, suspensions and mixes outside the boundaries of their ignition shall be maintained.
