In pursuance of articles 13 and 14 of the Law of Ukraine "About providing organization-legal conditions of social protection of the orphan children and children deprived of parent care", article 4 of the Law of Ukraine "About bodies and services for children and special facilities for children", Procedure Item 16 agencies of guardianship and cares of the activities connected with protection of the rights of the child approved by the resolution of the Cabinet of Ministers of Ukraine of 24.09.2008 of N 866, of Item 8 of the Procedure of activities for adoption and implementation of the supervision of observance of the rights of the adopted children approved by the resolution of the Cabinet of Ministers of Ukraine of 08.10.2008 of N 905, and for the purpose of ensuring protection of the rights and legitimate interests of orphan children, the children deprived of parent care, the children who appeared in difficult vital circumstances, creations of conditions for their device in families of citizens of PRIKAZYVAYU:

1. Approve applied:

Procedure for maintaining the Single electronic databank about the orphan children and children deprived of parent care, and family of potential adoptive parents, guardians, custodians, adoptive parents, parents tutors;

Procedure for maintaining the Single electronic databank of children who appeared in difficult vital circumstances;

form of electronic accounting and statistical card of the child;

form of electronic registration card of candidates for adoptive parents.

2. Declare invalid the order of the Ministry of Ukraine for family, youth and sport of 28.04.2007 N 1386 "About maintaining the Single electronic databank of orphan children, the children deprived of parent care, and the citizens wishing to take them on education, and the Single electronic databank of the children who appeared in difficult vital circumstances", registered in the Ministry of Justice of Ukraine 05.06.2007 for N 582/13849.

3. To State department on adoption and protection of the rights of the child to submit this order on state registration in the procedure established by the legislation.

4. To impose control over the implementation of this order on the deputy minister Kondratyuk T. V.
