1. Approve:

form of the individual program of rehabilitation of the disabled person issued by federal state institutions of medico-social examination according to the appendix N 1;

form of the individual program of rehabilitation of the handicapped child issued by federal state institutions of medico-social examination according to the appendix N 2;

procedure for development and implementation of the individual program of rehabilitation of the disabled person (handicapped child) according to the appendix N 3.

2. Declare invalid the order of the Ministry of Health and Social Development of the Russian Federation of November 29, 2004 N 287 "About approval of form of the individual program of rehabilitation of the disabled person issued by federal institutions of medico-social examination" (it is registered in the Ministry of Justice of the Russian Federation on December 22, 2004 N 6226).

3. This order becomes effective since October 1, 2008.
