According to article 15 of the Law of Ukraine "About insurance" and the Regulations on State commission on regulation of the markets of financial services of Ukraine approved by the Presidential decree of Ukraine of 04.04.2003 No. 292, State commission on regulation of the markets of financial services of Ukraine by POSTANOVILA:

1. Approve:

form of the message of the insurance and/or reinsurance nonresident broker of intention to perform activities in the territory of Ukraine it (is applied);

2. Consider the message of the insurance and/or reinsurance nonresident broker on intention to perform activities in the territory of Ukraine (further - the message) the statement for entering of information on him into the state register of insurance and reinsurance brokers in case of implementation by the insurance and/or reinsurance nonresident broker of activities in the territory of Ukraine through its permanent mission registered as the taxpayer according to the legislation of Ukraine and entered in the state register of insurance or reinsurance brokers as of 16.05.2008 according to Regulations on registration of insurance and reinsurance brokers and maintaining the state register of insurance and reinsurance brokers approved by the order of State commission on regulation of the markets of financial services of Ukraine of 28.05.2004 No. 736, registered in the Ministry of Justice of Ukraine 30.06.2004 for No. 801/9400.

3. To department of insurance regulation and supervision to check the received messages regarding compliance to the Procedure for filling of form of the message of the insurance and/or reinsurance nonresident broker on intention to perform activities in the territory of Ukraine, approved by this order.

The messages completed with violation of requirements of this Procedure return to persons who sent them, with reasons for the reason of return signed by the authorized officer Natskomfinuslug determined by the order of the Chairman Natskomfinuslug; information specified in such messages will not be promulgated.

4. To provide to department of state regulation and market development of financial services together with Legal department submission of this order to the Ministry of Justice of Ukraine for state registration.

5. Organizational and analytical ensuring work of the head to provide to management publication of this order in mass media after its state registration.

6. Control over the implementation of this order to assign to the member of the commission - the director of the department of insurance supervision.

Procedure for filling of form of the message of the insurance and/or reinsurance nonresident broker on intention to perform activities in the territory of Ukraine
