Based on the Presidential decree of the Azerbaijan Republic of August 19, 1998 "About application of the Law of the Azerbaijan Republic "About radiation safety of the population" the Cabinet of Ministers of the Azerbaijan Republic decides No. 758:

1. Approve "Form and procedure for creation of radiological and hygienic passports of the territories, the companies and organizations" and "Accounting treatment and monitoring doses of individual radiation" (are applied).

2. This resolution becomes effective from the date of signing.

Form and procedure for creation of radiological and hygienic passports of the territories, companies and organizations

1. This document is prepared on the basis of the Law of the Azerbaijan Republic "About radiation safety of the population" and establishes form of radiological and hygienic passports of the territories, the companies and organizations, the main objectives and the principles of their creation.

2. Creation of radiological and hygienic passports of the territories, the companies and organizations is recognized system of the state assessment of the main sources of ionizing radiation (natural and technogenic) and is guided by ensuring the radiation safety of the population connected with conditions of his life and activities.

3. Radiological and hygienic passports of the companies, organizations and territories using sources of ionizing radiation are kept in the appropriate order by body of the state sanitary inspection and the Ministry of Emergency Situations (appendices No. 1 and 2).

4. Shall be brought in the radiological and hygienic passport of the territories, the companies and organizations:

* assessment of radiation safety of the population (workers);

* data on the territory which was affected by the high level of ionizing radiation and on risk groups of the population (workers);

* the forecast of radiation situation for the territories and at the companies and in the organizations using sources of ionizing radiation;

* the recommendations necessary for planning and holding actions, and also decision making on ensuring radiation safety of the population (workers);

* efficiency analysis of the events held in connection with ensuring radiation safety of the population (workers).

5. The radiological and hygienic passport of the companies and organizations shall contain data on type and nature of the activities connected with use of sources of ionizing radiation, the place of conducting works on observance of requirements of hygiene standards and rules, probabilities of radiation accident and its scale, the prevention of accidents and readiness for liquidation of their effects, number of persons which underwent to the dose of radiation exceeding the established regulation and other necessary data on ready state and radiation safety.

6. In the radiological and hygienic passport of the territory features and extent of radioactive pollution of the territories, the analysis of doses of radiation to which separate national groups, and other data underwent owing to it according to conditions of radiation safety are specified.

7. The specified data shall be reflected in radiological and hygienic passports precisely and fully and to allow to estimate efficiency of actions for ensuring radiation safety.

8. Radiological and hygienic passports of the territories, the companies and organizations are constituted annually.
