Approve Rules of the individual (personified) accounting of insured persons in system of the national social insurance (are applied).

1. The these rules developed according to part two of article 5 of the Law of the Republic of Belarus of January 6, 1999 No. 230-Z "About the individual (personified) accounting in system of the national social insurance" establish general procedure for accounting of information about each insured person for the purposes of the national social insurance.
2. Conducting the individual (personified) accounting in system of the national social insurance (further - the personified accounting) and the methodical application guide of these rules are performed by Social Security Fund of the population of the Ministry of Labour and Social Protection (further - Social Security Fund of the population).
3. For the purposes of these rules the following main terms and their determinations are used:
biographical particulars – the information about insured person containing in the questionnaire of insured person;
registration of insured person – data processing, the compulsory insurance premiums represented to body of Social Security Fund of the population by the payer, opening of individual personal account of insured person (further – individual personal account) under certain insurance number and production to insured person of the insurance certificate of the national social insurance;
documents of the personified accounting (further – DPU) – the documents necessary for conducting the personified accounting.
Other terms are used in these rules in the values established by the legislation on the national social insurance, professional pension insurance, provision of pensions.
4. Treat DPU:
the questionnaire of insured person in the PU-1 form (further - the PU-1) form according to appendix 1;
data on acceptance and dismissal in the PU-2 form (further - the PU-2) form according to appendix 2;
individual data in the PU-3 form (further - the PU-3) form according to appendix 3;
the insurance certificate of the national social insurance in the PU-4 form (further - the certificate of social insurance) according to appendix 4;
request for submission of data from individual personal account of insured person in the PU-5 form (further - the PU-5) form. The PU-5 form affirms board of Social Security Fund of the population;
individual data on professional pension insurance in the PU-6 form (further - the PU-6) form according to appendix 5.
5. During conducting the personified accounting the accompanying documents providing accounting of the accepted and transferred DPU, the additional documents necessary for completion of the procedure of registration of insured person and forming of individual personal account, including special part of individual personal account, and also documents created based on the data containing in individual personal account, reported to the authorities performing payments for the national social insurance, and insured face can be created. Forms of these documents affirm board of Social Security Fund of the population.
6. These requirements belong to DPU specified in paragraphs the second or fourth and sixth or seventh item 4 of these rules.
7. DPU specified in paragraphs the second or fourth and seventh item 4 of these rules are filled and presented by the payer of fees in body of Social Security Fund of the population in the form of electronic documents through the information resource "Private Office of the Payer of Fees" placed on enterprise portal of Social Security Fund of the population.
With average number up to 5 people and legal successors for the reorganized employers filling and representation of DPU specified in paragraphs the second or fourth and seventh item 4 of these rules can be performed by payers of fees also electronically through the information resource "Private Office of the Payer of Fees".
Requirements to DPU electronically (in the form of electronic documents) and the basic rules of their filling and forming are established by board of Social Security Fund of the population.
8. The PU-1 form is filled and it is represented the payer of compulsory insurance premiums insured by person according to the data containing in the identity document, the passport or other document replacing it intended for trip abroad and issued by relevant organ of the state of civil accessory or the regular residence of the foreign citizen, stateless person or the international organization (further – the document for trip abroad).
