1. Approve the Accounting treatment, storages, registrations and certifications to persons receiving the government public assistance according to the Law of Ukraine "About the government public assistance to disabled people since the childhood and to handicapped children" which is attached.

2. Determine that the certificate which form is approved by the order of the Ministry of Labour and Social Policy of Ukraine of 28.10.2002 No. 549 "About approval of the certificate of person, the disabled person receiving the government public assistance since the childhood or the handicapped child" is valid in all territory of Ukraine before their exchange in accordance with the established procedure for new certificates.

3. To the Ministry of Labour and Social Policy of the Autonomous Republic of Crimea, head departments of work and social protection of the population of the Regional State Administrations, Head department of social protection of the population of the Kiev city state administration, Management of work and social protection of the population of the Sevastopol city state administration to begin issue according to addresses of disabled people with the childhood or handicapped children or their legal representatives (guardians) since January 1, 2008 by bodies of work and social protection of the population of the specified certificate to persons staying on the registry in these bodies as receivers of the government public assistance, according to the Law of Ukraine "About the government public assistance to disabled people since the childhood and to handicapped children".

4. Declare invalid the order of the Ministry of Labour and Social Policy of Ukraine of 28.10.2002 No. 549 "About approval of the certificate of person, the disabled person receiving the government public assistance since the childhood or the handicapped child".

5. To impose control of execution of this order on the deputy minister V. Dyachenko.

1. General provisions

1.1. The certificate of person, Ukraine receiving the government public assistance according to the Law "About the government public assistance to disabled people since the childhood and to handicapped children" (further - the certificate) (appendix 1), is the document confirming appointment to the disabled person since the childhood and to the handicapped child of the government public assistance according to the Law of Ukraine "About the government public assistance to disabled people since the childhood and to handicapped children" (further - the government public assistance).

1.2. Certificates are issued by structural divisions concerning social protection of the population of district, district public administrations, executive bodies in the cities of Kiev and Sevastopol city, district in the cities (except the cities of Kiev and Sevastopol) councils.

2. Registration and certification

2.1. The certificate of person, receiving the government public assistance is drawn up addressed to the disabled person since the childhood or the handicapped child to whom the government public assistance is appointed.

Data on the handicapped child or the disabled person since the childhood are filled in on page 1.

2.2. On the first page of the certificate of the persons having disability with shortcomings (defeat, disease) of hearing, sight and the musculoskeletal device, the reason of disability and group for disabled people is also specified (for example: "disabled person since the childhood of the I group on sight").

If the government public assistance is appointed to the handicapped child or the disabled person recognized as incapacitated person, then in the certificate the surname, name and middle name of his legal representative (guardian) (page 2 of the certificate) is also entered.

2.3. The certificate is issued to the disabled person since the childhood or to the handicapped child free of charge personally to the receiver of the government public assistance or the legal representative (guardian) of the handicapped child and the disabled person since the childhood recognized incapacitated or to other person representing the interests of the receiver based on notarially attested power of attorney in case of the presentation of the passport and the relevant documents confirming the personality under the signature in register of certificates.

2.4. In register of certificates are specified:

sequence number of record;

number of the certificate;

number, date of reception-transmission of certificates;

surname, name, middle name of the disabled person, handicapped child;

the document shown in case of receipt of the certificate, its details;

the signature and surname of person which issued the certificate;

the signature of person who received the certificate.

3. Accounting and storage of the certificate
