In pursuance of the resolution of the Cabinet of Ministers of Ukraine of December 21, 2005 "About approval of the Temporary procedure for issue of the certificate of origin of forest products and the timber made of them for implementation of export transactions" I ORDER to No. 1260:

1. Approve:

1.1. Form of the Certificate of origin of forest products and the timber made of them for implementation of export transactions (further - the Certificate) it (is applied);

1.2. The instruction concerning filling of form of the Certificate of origin of forest products and the timber made of them for implementation of export transactions (is attached).

2. To assign issue of Certificates to territorial authorities Goslesagentstva.

3. Goslesagentstva to provide to territorial authorities issue of Certificates to subjects of managing.

4. It is excluded according to the Order of the Ministry of environment protection and natural resources of Ukraine of 29.06.2022 No. 235

5. And labor protections (Voronyuk V. V.) to provide to the head of department of technical policy state registration of this order in the procedure established by the legislation.

6. To impose control of execution of this order on vice-chairmen according to job responsibilities.

The instruction concerning filling of form of the Certificate of origin of forest products and the timber made of them for implementation of export transactions

1. This Certificate is filled in in duplicate. One copy remains in the body which issued the Certificate. The second copy and the copy certified by the body which issued the Certificate is provided to the exporter. After customs clearance the copy of the exporter remains in customs.

2. The certificate is filled in in the following sequence:

2.1. Series - is filled according to Designation of abbreviation of areas for specifying of series of the Certificate of origin of forest products and the timber made of them for implementation of export transactions (appendix).

2.2. In line 1 "Forest user seller" are specified the name of the company which was carrying out realization of forest product in the domestic market, its location, area, the area, the name of the settlement, the street and house number, identification code on EGRPOU.

2.3. In line 2 "The characteristic of the purchased products" are specified type of forest product which was realized by the forest user seller in the domestic market, its breed, grade, the geometrical sizes, quantity, code according to UKTVED.

2.4. In the line 3 "Exporter" data on the business entity (legal entity or physical person) performing export of forest product, the name of the company (organization) or surname and initials of the physical person - the exporter, the location, identification code on EGRPOU are specified. In case exporter is the physical person - its passport data - number, series who and when issues also identification number (in the presence) are specified.

2.5. In line 4 "The basis of issue of the certificate" are specified the name of shipping documents according to which the forest product specified in line 2, their series, number and date of issue was acquired. 2.6. In line 5 "The characteristic of batch of forest product which is exported" the name of forest product which is exported breed, grade, the geometrical sizes, quantity, code according to UKTVED are specified. 3. In case of impossibility of filling of separate columns of the Certificate on the face in connection with considerable amount of information, its placement on reverse side is allowed. 4. After issue of the Certificate on the provided originals of accompanying documents the mark with indication of number and date of the issued Certificate for exception of possibility of issue according to the same documents of the Certificate repeatedly is performed. The copy transport or the rail waybill about acquisition leso-and timber, or the lesorubny ticket (for permanent forest users) is stored with the Certificate in the body which issued the Certificate.
