1. These rules are established by structure and procedure for representation by the organizer of gamblings of the data necessary for control of observance of requirements of the legislation on state regulation of activities for the organization and carrying out gamblings (further - data).
2. In these rules the basic concepts and terms determined by the Federal Law "About State Regulation of Activities for the Organization and Carrying Out Gamblings and about Modification of Some Legal Acts of the Russian Federation" are used (further - the Federal Law).
3. Data are represented monthly by the organizer of gamblings to governing body of gambling zone in the territory of which activities for the organization and carrying out gamblings are performed.
4. Data are represented on paper or in electronic form with use of information and telecommunication networks public, including Internet networks, in report type in form according to appendix No. 1 as of the last day of month under report no later than the last day of the month following for reporting in time.
Together with the report for December, March, June and September calculation of net assets value made according to the procedure, established by the Ministry of Finance of the Russian Federation is represented.
The report and calculation of net assets value represented on paper are signed by the head of the organizer of gamblings (the other person having the right to act without power of attorney on behalf of the legal entity) and certified by seal (in the presence of seal).
The report and calculation of net assets value represented in electronic form are signed by the strengthened qualified digital signature of the head of the organizer of gamblings (the other person having the right to act without power of attorney on behalf of the legal entity).
5. In the report names and types of all gambling institutions in which activities for the organization and carrying out gamblings, the organizations (including the organizer of gamblings), surnames, names and middle names of the individual entrepreneurs rendering the services accompanying gamblings (including branches and other places of implementation of activities for the organization and carrying out gamblings and to rendering the services accompanying gamblings are performed), and also their addresses (location) shall be specified.

6. 7. The declaration is on paper filled in with the organizer of gamblings on each gaming machine in 2 copies.
The first copy of the declaration is represented the organizer of gamblings in territorial authority of the Federal Tax Service in which the declared gaming machines are registered (further - territorial tax authority). The second copy of the declaration on paper is in gambling institution in which the declared gaming machine is established.
The declaration represented on paper is signed by the head of the organizer of gamblings (the other person having the right to act without power of attorney on behalf of the legal entity) and certified by seal (in the presence of seal).
The declaration represented in electronic form is signed by the strengthened qualified digital signature of the head of the organizer of gamblings (the other person having the right to act without power of attorney on behalf of the legal entity).
8. The declaration is submitted in territorial tax authority in case of each change of the name and version of the game programs installed in the gaming machine and also in case of value change technically of the pledged prize percent.
9. For the first time the declaration is submitted in territorial tax authority no later than 10 working days from the effective date of these rules, further in time - no later than 5 working days from the date of modification specified in Item 8 of these rules.

15. The organizer of gamblings annually represents the information about persons which has voting shares or share in the authorized capital of this organizer of gamblings in the amount of at least 10 percent and respectively directly and (or) indirectly can have significant effect on the solution of the questions carried to competence of general meeting of founders (participants) of this organizer of gamblings (further - the report), and also the documents confirming the specified data. In the report the number of voting shares (the share size) in the authorized capital of the organizer of gamblings which the shareholder (participant) has is specified.
