About approval of procedure for issue of the medical certificate about the child with disability aged to 18 years

 In pursuance of the Law of Ukraine "About the government public assistance to disabled people since the childhood and to handicapped children" PRIKAZYVAYU:

1. Approve procedure for issue of the medical certificate about the handicapped child aged up to 18 years it (is applied).

2. To the Minister of Health of the Autonomous Republic of Crimea, chiefs of managements of health care regional, Head department Kiev and managements of health care Sevastopol the city public administrations to provide accomplishment of this order.

3. To provide to the chief of the Center of medical statistics of MZ of Ukraine Golubchikov M. V. the Ministry of Health of the Autonomous Republic of Crimea and bodies of health care of local public administrations with samples of the accounting form 080/about "The medical certificate about the handicapped child aged up to 18 years".

4. Read out invalid the order MZ of Ukraine of 05.12.91 No. 175 "About the list of medical indications and procedure for issue of medical certificates to handicapped children from the childhood to 18 years".

Procedure for issue of the medical certificate about the child with disability aged to 18 years

1. Indications for establishment of disability at children are the pathological conditions arising in case of the congenital, heritable, got diseases and after injuries.

2. The list of the medical indications granting the right about receipt of the government public assistance to handicapped children aged up to 18 years is approved by the order of the Ministry of Health of Ukraine, the Ministry of Labour and Social Policy of Ukraine, the Ministry of Finance of Ukraine of 08.11.01 No. 454/471/516, registered in the Ministry of Justice of Ukraine 26.12.01 for No. 1073/6264.

3. Establishment at the child of medical indications for recognition by his disabled person aged up to 18 years is performed by the medical and advisory commissions (further - VKK) children's regional, multi-profile city hospitals, specialized hospitals, dispensaries where children stay on the dispensary registry and specialized treatment, the Ukrainian children's specialized hospital "OKHMATDET", the Ukrainian center of medical rehabilitation of children with organic defeat of nervous system, clinics of research establishments of the Ministry of Health of Ukraine and Academy of medical sciences of Ukraine after stationary or out-patient inspection.

4. In case of inspection in the specified institutions availability at the child of medical indications for recognition by his disabled person aged up to 18 years locates by the attending physician in primary medical documentation: to the medical record of the inpatient (f.003/about) or stories of development of the child (f.112/about) behind signatures of the attending physician, manager of department (polyclinic) and the deputy chief physician for medical part with specified according to the International classification of diseases (MKX-10) the diagnosis and code.

5. Similar record is entered in the statement from the medical record of the inpatient (f.027/about) or the advisory proceeding of the specialist (f.028/about) behind the signatures of the attending physician, manager of department (polyclinic), the deputy chief physician for medical part certified by seal of treatment-and-prophylactic institution.

6. The statement from the medical record of the inpatient (f.027/about) or the advisory proceeding of the specialist (f.028/about) are handed out to the child's parents, adoptive parents, the guardian or the custodian.

7. Recognition of the child aged up to 18 years by the disabled person and execution of the medical certificate about the handicapped child aged up to 18 years (f.080/about "The medical certificate about the handicapped child till 18 years" is attached) is performed by the medical and advisory commission of children's treatment-and-prophylactic institution for the place of residence of the child after its personal survey and in the presence of the statement from the medical record of the inpatient (f.027/about) or the advisory proceeding of the specialist (f.028/about), issued after stationary or out-patient inspection of the child in the nursery regional, multi-profile city hospitals, specialized hospitals, dispensaries where children stay on the dispensary registry and specialized treatment, the Ukrainian children's specialized hospital "OKHMATDET", the Ukrainian center of medical rehabilitation of children with organic defeat of nervous system or clinics of research establishments of the Ministry of Health, Academies of medical sciences of Ukraine and drawn up in that order.

8. Written reasons for recognition of the child by the disabled person aged up to 18 years are fixed in the history of development of the child (f.112/about) behind signatures of the chairman and members of the medical and advisory commission. 9. Data on registration and issue of the medical certificate on the child with disability aged up to 18 years are registered records of the conclusions of the medical and advisory commission (f.035/about).
