Based on paragraphs of third and ninth Item 12 of the Regulations on Social Security Fund of the population of the Ministry of Labour and Social Protection of the Republic of Belarus approved by the Presidential decree of the Republic of Belarus of January 16, 2009 No. 40, of Item 17, of Item part seven 21, of the paragraph of the fourth of part one of Item 38 of the Regulations on procedure for the providing with temporary disability benefits and on pregnancy and childbirth approved by the resolution of Council of Ministers of the Republic of Belarus of June 28, 2013 No. 569, board of Social Security Fund of the population of the Ministry of Labour and Social Protection of the Republic of Belarus DECIDES:

1. Approve Regulations on procedure for provision of data and information in case of purpose of temporary disability benefits and on pregnancy and childbirth it (is applied).

2. This resolution becomes effective since July 1, 2024.

Regulations on procedure for provision of data and information in case of purpose of temporary disability benefits and on pregnancy and childbirth

1. This Provision determines procedure for provision of the population of the Ministry of Labour and Social Protection by Social Security Fund (further – Fund) data and information to the payer of compulsory insurance premiums (further – the payer) for purpose of temporary disability benefits and by pregnancy and childbirth (further if other is not established, – benefits).

2. In this Provision terms in the values determined by the legislation on the national social insurance are used.

3. Provision of data and information in case of purpose of benefits is performed by city, district, district departments (sectors) in the cities regional (The Minsk city) managements, regional (To the Minsk city) managements of Fund (further – territorial authorities of Fund) in the cases which came since July 1, 2024 and later.

4. Interaction of the payer with Fund for provision of data and information is performed by means of:

the information resource "Private Office of the Payer of Fees" placed on enterprise portal of Fund;

the direct appeal to city, district, district departments (sectors) in the cities regional (The Minsk city) managements of Fund for the place of registration as the payer.

5. The payer for whom insured person works according to the employment contract (principle place of employment, on the terms of internal and (or) external combining jobs in the absence of principle place of employment or in case of finding of the receiver of benefit in child care leave before achievement of age by it 3 years on principle place of employment) and (or) performs works on the civil agreement which subject is rendering services performance of works and creation of intellectual property items, at the legal entity, the individual entrepreneur, the notary (further – the civil agreement) or is the physical person who is the owner of property (the participant, the member, the founder) of the legal entity and performing functions of his head (further – the founder), no later than two working days, from the date of the request of insured person for benefit, creates and sends to territorial authority of Fund inquiry for receipt of data on the periods of payment of compulsory insurance premiums and the size of daily average earnings for calculation of temporary disability benefit, pregnancy and to childbirth (further – request) in form according to appendix 1. The separate inquiry is sent by each civil agreement during which operation there came the case of temporary disability, pregnancy and childbirth.

Requirements to filling, structure and format of request form are established in appendix 2.

The request which is not conforming to the requirements established in part one of this Item is not accepted by territorial authority of Fund with indication of return reason.

The payer within two working days shall remove the cause specified in return create and send new inquiry to territorial authority of Fund.

6. The request form has two types: initial and canceling. When filling request form in the field of the detail "Form Type" the mark becomes.

The request is filled on each leaf of disability of the insured person working:

according to the employment contract – only the payer for whom insured person works on principle place of employment that is reflected in data on acceptance and dismissal in the PU-2 form (further – the PU-2) form;

as the founder – the payer at whom insured person is founder that is reflected in individual data in the PU-3 form (further – the PU-3) form, and has no principle place of employment according to the employment contract;

on the terms of combining jobs, in the absence of principle place of employment or in case of finding of the receiver of benefit in child care leave before achievement of age of 3 years by it on principle place of employment – the payer at whom insured person works at combining jobs conditions that is reflected in the PU-2 form;

under the civil agreement – the payer at whom insured person performs works on the civil agreement and in the PU-3 form reflects the amounts of remuneration under this civil agreement.

In case of need of adjustment of the data which are earlier provided in request the new request is filled (form type – initial).

In case of need of cancellation of the data which are earlier provided in request the request is filled (form type – canceling).

If in earlier sent request form the mistake was made (except for the details "Surname", "Own Name" and "Middle Name (if That Is Available)"), then the request with form type canceling shall be provided, in other cases the form type initial is filled.

7. When forming request the payer shall be guided by the following requirements:

7.1. the accounting room of the payer in Fund and accounting number of the payer correspond to urgent data, including in case of change of the location (residence), for date of representation of request;

7.2. start date of case of temporary disability, pregnancy and childbirth not earlier than July 1, 2024;

7.3. on the insurance number of the individual ledger account of insured person specified in request (further – ILS ZL) opens individual personal account within the individual (personified) accounting of insured persons in system of the national social insurance;
