About approval of the Instruction about accounting treatment, registration and uses of forms of insurance certificates, insurance policies and insurance certificates "Green card" on obligatory civil liability insurance of owners of vehicles
According to the Charter of the Belarusian bureau on transportation insurance approved by the Presidential decree of the Republic of Belarus of December 1, 1999 No. 701, I ORDER:
Approve the enclosed Instruction about accounting treatment, registration and uses of forms of insurance certificates, insurance policies and insurance certificates "Green card" on obligatory civil liability insurance of owners of vehicles.

1. This Instruction determines accounting treatment, registration and uses of forms of the insurance certificates, insurance policies and insurance certificates "Green card" applied in case of the conclusion of agreements of obligatory civil liability insurance of owners of vehicles.
2. Blank forms of the insurance certificate, the insurance policy and the insurance certificate "Green card" affirm the Belarusian bureau on transportation insurance in coordination with the Ministry of Finance of the Republic of Belarus.
The insurance certificates, insurance policies and insurance certificates "Green card" applied to the conclusion of agreements of obligatory civil liability insurance of owners of vehicles are forms of the strict reporting and are subject to obligatory registration in the Belarusian bureau on transportation insurance.
3. Production and use of forms of the insurance certificates, insurance policies and insurance certificates "Green card" which are not registered in accordance with the established procedure is not allowed.
For production of forms of insurance certificates (insurance policies, insurance certificates) the insurance company which is the member of the Belarusian bureau on transportation insurance files in bureau petition (in duplicate by each type of forms) the established sample according to appendix 1 to this Instruction in which it specifies remaining balance of unused forms, expected expense of forms in future period, the necessary number of forms and the company at which forms will be made.
4. The bureau within five working days considers this application, assigns to circulation series and numbers about what it informs insurance company (by the direction of the second copy of the application with mark on assignment of series and numbers to insurance certificates (insurance policies, insurance certificates), and also the company at which forms will be made.
At the same time insurance companies - associated members of bureau have no right to production, registration and use of forms of insurance certificates "Green card".
5. After receipt of the made forms the insurance company files in bureau petition for their registration (by each type of forms) in form according to appendix 2 to this Instruction.
The statement for registration of forms of insurance certificates (insurance policies, insurance certificates) is constituted in duplicate. Are enclosed to the application: the verified copy of commodity-transport delivery note on leave of forms of the strict reporting of the established sample, the original of the first form of the insurance certificate (the insurance policy, the insurance certificate) of the made circulation and delivery note on transfer of bureau of the original of the form of the insurance certificate (the insurance policy, the insurance certificate).
The bureau within five working days considers this application, checks compliance of the made forms of insurance certificates (insurance policies, insurance certificates) to the approved sample, color scale, and also the appropriated series and numbering and makes their registration about what informs insurance company by the direction of one copy of the application with mark on registration.
6. The forms of insurance certificates (insurance policies, insurance certificates) which are not corresponding to the established sample, the color scale appropriated to series or numbering are not subject to registration.
7. Production of insurance certificates (insurance policies, insurance certificates) of the incomplete circulation allocated to bureau of insurance company is not allowed.
8. Insurance certificates, policies of boundary insurance and insurance certificates "Green card" are written out by advance holders in the chronological sequence in ascending order of their numbers.
9. Receipt, accounting and write-off of insurance certificates (insurance policies, insurance certificates) are made according to the regulations on procedure for use of forms of the strict reporting approved by the Ministry of Finance of the Republic of Belarus.
