1. Approve the enclosed forms intended for collection of administrative data:

1) the monthly report on appointment and payment of the government address public assistance according to appendix 1 to this order;

2) the monthly report on appointment and payment of the government address public assistance in type of the unconditional monetary assistance according to appendix 2 to this order;

3) the monthly report on appointment and payment of the government address public assistance in type of the caused monetary assistance according to appendix 3 to this order;

4) the quarterly report on appointment and payment of the government address public assistance according to appendix 4 to this order;

5) the quarterly report on appointment and payment of the government address public assistance in type of the unconditional monetary assistance according to appendix 5 to this order;

6) the quarterly report on appointment and payment of the government address public assistance in type of the caused monetary assistance according to appendix 6 to this order;

7) the monthly report on receivers of the appointed government address public assistance according to appendix 7 to this order;

8) the monthly report on receivers of the appointed government address public assistance in type of the unconditional monetary assistance according to appendix 8 to this order;

18) the quarterly report on appointment and payment of monthly additional payment on each child aged from one year up to six years inclusive according to appendix 18 to this order;

19) the monthly report of the assistant on support of the social contract according to appendix 19 to this order;

20) the monthly report of the consultant for social work on support of the social contract according to appendix 20 to this order.

2. Recognize invalid some orders of the Ministry of Labour and Social Protection of the population of the Republic of Kazakhstan according to appendix 21 to this order.

3. To provide to department of the public assistance of the Ministry of Labour and Social Protection of the population of the Republic of Kazakhstan in the procedure established by the legislation of the Republic of Kazakhstan:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) placement of this order on Internet resource of the Ministry of Labour and Social Protection of the population of the Republic of Kazakhstan after its official publication;

3) within ten working days after state registration of this order in the Ministry of Justice of the Republic of Kazakhstan submission to Department of legal service of the Ministry of Labour and Social Protection of the population of the Republic of Kazakhstan of data on execution of the actions provided by subitems 1) and 2) of this Item.

4. To impose control of execution of this order on the supervising vice-Minister of Labour and Social Protection of the population of the Republic of Kazakhstan.

5. This order becomes effective since July 1, 2023 and is subject to official publication.

Chapter 1. General provisions

1. This explanation determines single requirements for forming of report form about appointment and payment of the government address public assistance (further – report form).

2. The main objective of maintaining this form of the report is implementation of monitoring of appointment and payment of the government address public assistance.

3. The monthly report on appointment and payment of the government address public assistance, in form approved by appendix 1 to this order is created is automated by "Center of Development of Manpower" joint-stock company and it is represented in the Ministry of Labour and Social Protection of the population of the Republic of Kazakhstan monthly to the 10th following the accounting period.

The report form is created is automated as of the first following the accounting period by means of the automated information system of the Ministry of Labour and Social Protection of the population.
