About approval of procedure for provision to separate categories of persons of services in complex rehabilitation (training)

Cabinet of Ministers of Ukraine decides:

1. Approve the Procedure for provision to separate categories of persons of services in complex rehabilitation (training) which is applied.

Procedure for provision to separate categories of persons of services in complex rehabilitation (training)

1. This Procedure determines procedure for granting to persons with disability and/or to children with disability, and/or to children under three years who belong to risk group concerning receipt of disability (for the purpose of the prevention of disability) (further - receivers), of services in complex rehabilitation (training).

2. Services of complex rehabilitation (training) in rehabilitation organization which performs activities according to the Law of Ukraine "About rehabilitation of persons with disability in Ukraine" and to other regulatory legal acts in the sphere of rehabilitation (abilitatsionny) are provided to the receiver.

3. For receipt of services in complex rehabilitation (training), in particular for the purpose of the prevention of disability, in rehabilitation organization of the state or municipal pattern of ownership (further - organization) the receiver or his legal representative files the following documents:

1) the statement for provision of services on complex rehabilitation (training);

2) the individual program of rehabilitation issued by medico-social commission of experts, the medical and advisory commission of treatment and prevention facility (for children with disability);

3) the conclusion of the medical and advisory commission of treatment and prevention facility (for children under three years who belong to risk group concerning receipt of disability) with recommendations about passing of complex rehabilitation (training) in organization;

4) the passport of the citizen of Ukraine, the certificate of birth (for children with disability, children under three years who belong to risk group concerning receipt of disability) or other identity document;

5) the document certifying registration of physical persons in the State register of physical persons taxpayers;

6) the education document (for the receivers needing employment rehabilitation);

7) the statement from the medical record of the ambulatory (stationary) patient (form No. 027/about);

8) the certificate of capture on accounting of internally displaced persons (for receivers who are internally displaced persons).

Statements for provision of services on complex rehabilitation (training) with necessary documents are adopted from receivers or their legal representatives by structural divisions concerning social protection of the population district, district in Kiev and Sevastopol of the state administration, executive body city (the cities of republican and regional value), district in the city (in case of its creation) council, except of Kiev and Sevastopol in the place of their registration (further - local body). Since January 1, 2021 statements with necessary documents for receipt of services in complex rehabilitation (training) are adopted from receivers or their legal representatives of local authorities of the district state administrations only in case of sending them electronically (through the official website of Minsotspolitiki, information systems of executive bodies and local government bodies integrated with it or the Single state web portal of electronic services).

Statements with necessary documents for receipt of services in complex rehabilitation (training) are also adopted by authorized officers of executive body of village, settlement, city council of the corresponding territorial community (further - officials of executive body) and officials of the center of provision of administrative services which transfer them within three working days from the date of their receipt to local body.

Till July 1, 2021 the official of executive body or center of provision of administrative services, adopts the statement with necessary documents for receipt of services in complex rehabilitation (training), if technically possible registers the statement and creates electronic case (fills the corresponding electronic forms with use of information systems of Minsotspolitiki) and no later than the next working day directs electronic case to the relevant local organ means of electronic communication through information systems of Minsotspolitiki with observance of requirements of the Laws of Ukraine "About electronic documents and electronic document management", "About electronic confidential services", "About information security in information and telecommunication systems".

Since July 1, 2021 statements with necessary documents for receipt of services in complex rehabilitation (training) in organization are adopted by officials of executive body or center of provision of administrative services only with forming of electronic case.

Statements with necessary documents for receipt of services in complex rehabilitation (training) (after forming of electronic case) in paper form are transferred in organization to the relevant local organ at least every two weeks.

The local body checks availability of the submitted documents.

4. Receipt date of the documents specified in Item 3 presents of the Procedure which are registered in the relevant magazine is considered day of the appeal of the receiver or his legal representative to local body.

5. Local body within five working days:

informs the receiver or his legal representative on the made decision concerning the direction on complex rehabilitation (abilitation);

introduces in centralized databank on disability problems information on the direction of the receiver on complex rehabilitation (abilitation);

directs to organization the notification on the direction of the receiver to complex rehabilitation (abilitation) with mark about entering of information on the receiver into centralized databank on problems of disability and the copy of the documents specified in Item 3 presents of the Procedure.
