1. Approve on representation of the Ministry of Health:
1.1. form of the state statistical reporting 1 children (Ministry of Health) "The report on medical care to children" (is attached) and to enact it starting with the report for 2022;
1.2. Instructions for filling of form of the state statistical reporting 1 children (Ministry of Health) "The report on medical care to children" (are applied) and to enact them starting with the report for 2022.
2. Extend the form of the state statistical reporting specified in subitem 1.1 of Item 1 of this resolution to the state organizations of health care (except leaders only advisory acceptance) subordinated to the Ministry of Health, local executive and administrative organs; the republican state bodies (except the Ministry of Health) having the state organizations of health care in the subordination.
3. Recognize invalid:
the resolution of National Statistical Committee of the Republic of Belarus of October 25, 2018 No. 115 "About approval of form of the state statistical reporting 1 children (Ministry of Health) "The report on medical care to children" and instructions for its filling";
subitem 1.33 of Item 1 of the resolution of National Statistical Committee of the Republic of Belarus of November 5, 2019 No. 113 "About change of resolutions of National Statistical Committee of the Republic of Belarus";
the resolution of National Statistical Committee of the Republic of Belarus of October 16, 2020 No. 93 "About change of the resolution of National Statistical Committee of the Republic of Belarus of October 25, 2018 No. 115";
