1. Approve enclosed:

1) the Unified form of the statement which is drawn up by results of holding planned (unplanned) action of the state supervision (control) for observance by the subject of managing of requirements of the legislation, regulations and standards in the sphere of rail transport;

2) instruction Form on elimination of the violations revealed during planned (unplanned) action of the state supervision (control) for observance by the subject of managing of requirements of the legislation, regulations and standards in the sphere of rail transport.

2. Declare invalid the order of the Ministry of infrastructure of Ukraine of June 22, 2020 No. 365 "About approval of the unified form of the statement which is drawn up by results of holding planned (unplanned) action of the state supervision (control) for observance by the subject of managing of requirements of the legislation, regulations and standards in the sphere of rail transport, and instruction form on elimination of the violations revealed during planned (unplanned) action of the state supervision (control) for observance by the subject of managing of requirements of the legislation, regulations and standards in the sphere of rail transport", No. 813/35096 registered in the Ministry of Justice of Ukraine on August 21, 2020.

3. (A. Zadorozhny) to provide to department of road transportations and safety on transport submission of this order on state registration in the Ministry of Justice of Ukraine in accordance with the established procedure.

4. To provide to the sector of external communications placement of this order on the official website of the Ministry of infrastructure of Ukraine.

5. This order becomes effective from the date of its official publication.
