1. Establish forms:

analysis results of seeds of agricultural plants according to appendix 1;

notifications on refusal in issue (prolongation of effective period) of the certificate of quality of seeds of agricultural plants according to appendix 2.

2. Recognize invalid:

the resolution of the Ministry of Agriculture and Food of the Republic of Belarus of November 19, 2013 No. 61 "About establishment of document forms";

subitem 1.2 of Item 1 of the resolution of the Ministry of Agriculture and Food of the Republic of Belarus of October 4, 2017 No. 49 "About modification and amendments in some resolutions of the Ministry of Agriculture and Food of the Republic of Belarus concerning seed farming".

3. This resolution becomes effective after its official publication.
