About approval of forms of the statements which are drawn up by results of unscheduled inspection on observance of requirements of the legislation on electronic communications and radio-frequency range

According to Articles 13, of 54, of 70, 78 Laws of Ukraine "About electronic communications", the National commission performing state regulation in spheres of electronic communications, radio-frequency range and rendering services of mail service DECIDES:

1. Approve enclosed:

form of the act of carrying out unscheduled inspection of observance of requirements of the legislation on electronic communications and radio-frequency range;

form of the act of factual determination of illegal transfer or provision to use of individual rights to use of radio-frequency range to other legal entity or physical person;

form of the act of repeated unmotivated failure of the user of radio-frequency range in conducting check by regulatory body for compliance with law concerning use of radio-frequency range;

form of the act of further violation by the user of radio-frequency range of conditions of the license and/or other obligations, the stipulated in Article 49 Laws of Ukraine "About electronic communications";

form of the act of failure to meet requirements of the order of regulatory body;

form of the act of failure to meet requirements of the instruction of regulating authority of elimination of violations of the conditions specified in radio-frequency assignment or conditions which are applied to rights to use by radio-frequency range according to the Law of Ukraine "About electronic communications", including not elimination of radio interferences;

form of the act of further violation by the user of resource of numbering of the requirements determined by article 79 of the Law of Ukraine "About electronic communications";

form of the act of failure to meet requirements of the order of the national regulator of elimination of violations of conditions of use of numbering resource according to article 79 of the Law of Ukraine "About electronic communications";

form of the act of repeated unmotivated failure of the user of resource of numbering in conducting check by regulatory body for compliance with law concerning use of numbering resource.

2. In accordance with the established procedure to submit to department of the state supervision this resolution on state registration to the Ministry of Justice of Ukraine.

3. The resolution becomes effective from the date of its official publication.
