According to article 16-3 of the Law of Ukraine "About transport", to Article part fifteen 4, of Article part two 5, article 6 of the Law of Ukraine "About the basic principles of the state supervision (control) in the field of economic activity", article 19 of the Law of Ukraine "About licensing of types of economic activity", to the paragraph to the twelfth the subitem 38 of item 4 of the Regulations on the Ministry of infrastructure of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of June 30, 2015 No. 460, to the Technique of development of the unified forms of the statements which are drawn up by results of holding planned (unplanned) actions of the state supervision (control), Ukraine approved by the resolution of the Cabinet of Ministers of May 10, 2018 No. 342, I ORDER:

1. Approve enclosed:

1) the Unified form of the statement which is drawn up by results of holding planned (unplanned) action of the state supervision (control) for observance by the subject of managing of requirements of the legislation in the field of public conveyance, dangerous goods, dangerous wastes by sea and river transport and safety on sea and river transport (except the sphere of safety of navigation of vessels of the fleet of fish industry);

2) instruction Form on elimination of the violations revealed during planned (unplanned) action of the state supervision (control) in spheres of public conveyance, dangerous goods, dangerous wastes by sea and river transport and safety on sea and river transport (except the sphere of safety of navigation of vessels of the fleet of fish industry);

2. Declare invalid the order of the Ministry of infrastructure of Ukraine of April 15, 2020 No. 235 "About approval of the unified form of the statement which is drawn up by results of holding planned (unplanned) action of the state supervision (control) for observance by the subject of managing of requirements of the legislation in the field of public conveyance, dangerous goods, dangerous wastes by sea and river transport and safety on sea and river transport (except the sphere of safety of navigation of vessels of the fleet of fish industry), and other forms of administrative documents", registered in the Ministry of Justice of Ukraine on August 03, 2020 for No. 740/35023.

3. To management of sea and river transport (Ilyasevich Ya.) provide submission of this order on state registration in the Ministry of Justice of Ukraine in accordance with the established procedure.

4. To provide to the sector of external communications promulgation of this order on the official website of the Ministry of infrastructure of Ukraine.

5. This order becomes effective from the date of its official publication.

6. To impose control of execution of this order on the deputy minister Vaskov Yu.
