According to Item 2 of the resolution of the Cabinet of Ministers of Ukraine of November 17, 2021 No. 1201 "About modification of the inventory (including medicines, medical products and/or the medical equipment), the actions, necessary for accomplishment, directed to prevention of origin and distribution, localization and liquidation of flashes, epidemics and pandemics of sharp respiratory disease of COVID-19, of the transaction SARS-CoV-2, caused by coronavirus on import of which to customs area of Ukraine and/or transaction on delivery of which on customs area of Ukraine are exempted from taxation on value added", Item 8 of the Regulations on the Ministry of Health of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of March 25, 2015 No. 267 (in edition of the resolution of the Cabinet of Ministers of Ukraine of January 24, 2020 to No. 90), I ORDER:

1. Approve the Procedure for confirmation of target use of the equipment for production of the vaccines and medicines necessary for implementation of the actions directed to prevention of origin and distribution, localization and liquidation of flashes, epidemics and pandemics of sharp respiratory disease of COVID-19, of SARS-CoV-2, caused by coronavirus which is applied.

2. To management of provision of medicines (Zadvornykh I. S.) provide submission of this order in the procedure established by the legislation on state registration in the Ministry of Justice of Ukraine.

3. I reserve control of execution of this order.

4. This order becomes effective from the date of its official publication.

Procedure for confirmation of target use of the equipment for production of the vaccines and medicines necessary for implementation of the actions directed to prevention of origin and distribution, localization and liquidation of flashes, epidemics and pandemics of sharp respiratory disease of COVID-19, of SARS-CoV-2 caused by coronavirus

1. This Procedure determines the mechanism of confirmation of target use of goods - the equipment for production of the vaccines and medicines necessary for implementation of the actions directed to prevention of origin and distribution, localization and liquidation of flashes, epidemics and pandemics of sharp respiratory disease of COVID-19, of SARS-CoV-2 caused by coronavirus (further - the equipment for production of medicines) included in the Inventory (including medicines, medical products and/or the medical equipment), the actions, necessary for accomplishment, directed to prevention of origin and distribution, localization and liquidation of flashes epidemics and pandemics of sharp respiratory disease of COVID-19, of SARS-CoV-2, of transaction caused by coronavirus on import of which to customs area of Ukraine and/or transaction on delivery of which on customs area of Ukraine are exempted from taxation on value added, No. 224 approved by the resolution of the Cabinet of Ministers of Ukraine of March 20, 2020 (further - the resolution of the Cabinet of Ministers of Ukraine of March 20, 2020 No. 224) which transactions on import to customs area of Ukraine and/or transaction on delivery of which on customs area of Ukraine by the subjects of managing performing economic activity on production of medicines in the territory of Ukraine (further - the applicant) are exempted from the taxation by the value added tax.

2. Action of this Procedure extends to the equipment for production of the medicines included in the List of the medicines necessary for implementation of the actions directed to prevention of origin and distribution, localization and liquidation of flashes, epidemics and pandemics of sharp respiratory disease of COVID-19, of SARS-CoV-2, caused by coronavirus No. 1012 approved by the resolution of the Cabinet of Ministers of Ukraine of September 22, 2021.

3. For which confirmation of target use of the equipment for production of medicines of transaction on import to customs area of Ukraine and/or transaction on delivery of which on customs area of Ukraine are exempted from taxation on value added, the applicant submits to the Ministry of Health the application constituted in any form concerning the equipment for production of medicines for import to customs area of Ukraine and delivery on customs area of Ukraine in which information relatively is specified:

1) name (trade name);

2) code according to UKT foreign trade activities;

3) names of the equipment given in the Section "The Equipment for Production of the Vaccines and Medicines Necessary for Implementation of the Actions Directed to Prevention of Origin and Distribution, Localization and Liquidation of Flashes, Epidemics and Pandemics of Sharp Respiratory Disease of COVID-19, of SARS-CoV-2 Caused by Coronavirus" of the resolution of the Cabinet of Ministers of Ukraine of March 20, 2020 No. 224, to which there corresponds the equipment for production of medicines specified in the statement which is planned for import to customs area of Ukraine and delivery on customs area of Ukraine;

4) quantities of units;

5) costs (without the value added tax, UAH);

6) use purposes;

7) countries of source;

8) names and locations of the producer;

9) serial number (in the presence).

4. Are enclosed to the application:

1) the copies certified in accordance with the established procedure:

external economic contract (contract) or purchase agreement of the equipment for production of medicines between the applicant and producer and/or supplier;
