1. To territorial authorities of Federal Service for the Oversight of Consumer Protection and Welfare:

1.1. Organize registration of persons, victims of radiative effects and the undergone to radiation exposure as a result of Chernobyl and other radiation catastrophic crashes and incidents according to this order;

1.2. Provide maintaining, regional banks of data on persons, victims of radiative effects and undergone to radiation exposure as a result of Chernobyl and other radiation catastrophic crashes and incidents living in the territory of appropriate subject of the Russian Federation, having recovered this information since July 1, 2004;

1.3. Organize information transfer in the established form in the Federal databank of "Federal Center of Hygiene and Epidemiology" Federal state healthcare institution of Federal Service for the Oversight of Consumer Protection and Welfare.

2. To "The centers of hygiene and epidemiology" of Federal Service for the Oversight of Consumer Protection and Welfare to provide to federal state healthcare institutions conducting sanitary and epidemiologic examination of working conditions of workers and conditions of impact of radiation exposure on the population in the conditions of radiation accident for the purpose of registration of victims of radiative effects and undergone to radiation exposure in the relevant territorial administrations.

3. To "Federal Center of Hygiene and Epidemiology" federal state healthcare institution of Federal Service for the Oversight of Consumer Protection and Welfare:

3.1. Develop and approve till 01.01.2007 regulations on the Federal databank on persons, victims of radiative effects and the undergone to radiation exposure as a result of Chernobyl and other radiation catastrophic crashes and incidents;

3.2. Provide maintaining the Federal databank on persons, victims of radiative effects and undergone to radiation exposure as a result of Chernobyl and other radiation catastrophic crashes and incidents, having recovered this information since July 1, 2004;

3.3. Annually till April 1, and also on requests to submit in the Federal Service reports on registered persons, victims of radiative effects and undergone to radiation exposure.

4. To "Federal Center of Hygiene and Epidemiology" federal state healthcare institution together with Federal state institution of science "St. Petersburg research institution of radiation hygiene of professor P. V. Ramzayev" of Federal Service for the Oversight of Consumer Protection and Welfare:

4.1. Prepare and provide to the Federal Service till 01.04.2007 the project methodical to the recommendation about maintaining regional and Federal databanks on persons, victims of radiative effects and the undergone to radiation exposure as a result of Chernobyl and other radiation catastrophic crashes and incidents;

4.2. Develop software for maintaining databanks.

5. Determine what in case of registration of information about persons, victims of radiative effects and the undergone to radiation exposure as a result of Chernobyl and other radiation catastrophic crashes and incidents is performed on the sample given in appendix N 1, and recommendations about filling (appendix N 2).

6. To impose control over the implementation of this order on the deputy manager of Federal Service for the Oversight of Consumer Protection and Welfare L. P. Gulchenko.
