About approval of form of the tax declaration of the payer of the single tax of the third group for action of warlike, emergency state in Ukraine

According to Item 296.9 of Article 296 of Chapter 1 of the Section XIV, subitems 9. 4, 9.6, 9.7 of Item 9 of subsection 8 of the Section XX "Transitional provisions" of the Tax code of Ukraine and the subitem 5 of item 4 of the Regulations on the Ministry of Finance of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of August 20, 2014 No. 375, I order:

1. Approve form of the tax declaration of the payer of the single tax of the third group for action of warlike, emergency state in Ukraine which is applied.

2. Determine that the tax declaration of the payer of the single tax of the third group on the flat tax rate in the amount of 2 percent of the income, moves in the form approved by this order.

3. In accordance with the established procedure to provide to department of tax policy of the Ministry of Finance of Ukraine:

submission of this order on state registration in the Ministry of Justice of Ukraine;

promulgation of this order.

4. This order becomes effective from the date of its official publication.

5. To impose control of execution of this order on the Deputy Minister of Finance of Ukraine Vorobey S. I. and chairman of the State Tax Administration of Ukraine.
