For the purpose of ensuring accomplishment of Items 7.3.10 - 7.3.12 of "The state program (Electronic Azerbaijan) on development of communication and information technologies for 2005 - 2008 in the Azerbaijan Republic", the President of the Azerbaijan Republic approved by the Order of October 21, 2005 1055, the Cabinet of Ministers of the Azerbaijan Republic decides No.:

1. Approve "Rules of implementation of the Electronic Card of Health system it (is applied).

2. This resolution becomes effective from the date of signing.

Rules of implementation of system of "The electronic card of health"

1. General provisions

1.1. These rules are prepared based on Items 7.3.10 7.3.12 of "The state program (Electronic Azerbaijan) on development of communication and information technologies for 2005 - 2008 in the Azerbaijan Republic", the President of the Azerbaijan Republic approved by the Order of October 21, 2005 No. 1055, and establish rules of creation of the Electronic Card of Health system (further - the card of health) and the medical electronic register.

1.2. Implementation of system of the card of health allows to enhance by means of creation of the corresponding electronic registers the control system of the state of health of citizens and promptly creation of databank about health of the personality.

1.3. The system of the card of health implemented by the Ministry of Health of the Azerbaijan Republic is the electronic information system comprising personal, medical and insurance data.

1.4. Information which arrived in system of the card of health gathers in Information center of the Ministry of Health of the Azerbaijan Republic.

1.5. Information existing in the card is unpacked and changes by means of the special device adapted for the computer in machines of emergency medical service, in departments of registration of hospitals, laboratories, drugstores, etc.

1.6. The data which are available in system of the card of health (the clinical record, medicines used at the moment, inoculations, medical insurance, etc.) give the chance to the card holder quickly and to be examined precisely.

2. Structure of system of the card of health

2.1. The system of the card of health consists of software of the Central information base and the relevant medical institutions, the special unpacking device of the personal computer of organization and the electronic card supplied with the microprocessor chip and which is not giving in to counterfeit.

2.2. The employee to whom the management of medical institution grants the right of work with the program, entering the program, performs operations with the card of the owner. Data on the performed operations are financed in the program.

3. Creation and maintaining register of system of the card of health

3.1. For the purpose of accounting of the state of health of citizens in Information center of the Ministry of Health of the Azerbaijan Republic the electronic register of bank of cards of health is created.

3.2. The register and the map of health include the following data:

* about the personality;

* about medical institution;

* about immunization;

* other medical data established by the legislation.

4. Rules of production and issue of cards of health

4.1. Cards of health are manufactured in the organizations established by the Ministry of Health of the Azerbaijan Republic provided with the special equipment and are distributed to medical institutions for issue to physical persons.

4.2. The system of cards of health is constituted based on the information provided by medical institutions.

4.3. In case of impossibility of updating of results of inspection on the place in medical institution, these changes go to Information center and subsequently medical institution join in the card of the owner.

4.4. To the persons who lost or damaged the card of health new maps and information containing in Information center are made, registers in memory of the new card. The lost or damaged cards turn on in the list of the cards forbidden for use, go to Information center and are blocked.

5. Receipt of information on the card holder of health and its privacy

5.1. Login for the purpose of receipt of information on the card holder of health and carrying out transactions in system is performed only from the corresponding permission of management of medical institution.

5.1-1. The data in the electronic card of health concerning filling of the passport and the identity certificate join in "The interdepartmental automated "Entrance Exit and Registration" information retrieval system.

5.1-2. for placement on the electronic medium (chip) of the individual ID card of data of the citizen of the Azerbaijan Republic who did not reach 15-year age according to its electronic card of health, the State register of the population of the Azerbaijan Republic is transferred to the automated information system of registration.

5.2. Distribution of the confidential information established by the law is forbidden.

6. Supply and financing of system of the card of health

6.1. For activities of system of cards of health medical institutions are supplied with the equipment and other accessories.

6.2. The means which are required for implementation of system of cards of health are allocated within the means provided by centralized expenses of the Section "Expenses on Health Care" of the government budget by the corresponding years.
