1. Approve on representation of the Ministry of Health:
1.1. form of the state statistical reporting 1 psychiatry (Ministry of Health) "The report on diseases of mental disturbances, disorders of behavior (except the diseases connected with the use of psychoactive agents) and the contingents of observed patients" (is attached) and to enact it starting with the report for 2021;
1.2. Instructions for filling of form of the state statistical reporting 1 psychiatry (Ministry of Health) "The report on diseases of mental disturbances, disorders of behavior (except the diseases connected with the use of psychoactive agents) and the contingents of observed patients" (are applied) and to enact them starting with the report for 2021.
2. Extend the form of the state statistical reporting specified in subitem 1.1 of Item 1 of this resolution to the state organizations of health care providing medical care in stationary and out-patient conditions, subordinated to republican state bodies, local executive and administrative organs.
3. Recognize invalid:
