Some questions of insurance payments, the stipulated in Article 39 Laws of Ukraine "About protection of the population against infectious diseases"

Cabinet of Ministers of Ukraine decides:

1. Determine that the Pension fund of Ukraine of Ukraine carries out insurance payments, stipulated in Item 1 parts two of article 39 of the Law of Ukraine "About protection of the population against infectious diseases", to health workers of institutions of health care which established group of disability owing to disease of sharp respiratory disease of COVID-19, of SARS-CoV-2, caused by coronavirus provided that such disease is connected with execution of professional obligations in the conditions of the increased risk of infection before entry into force of the Law of Ukraine "About modification of article 39 of the Law of Ukraine "About protection of the population against infectious diseases" concerning settlement of single questions of social protection of health workers, victims of koronavirusny disease (COVID-19)" and which did not receive insurance payments.

2. Bring in the resolution of the Cabinet of Ministers of Ukraine of June 17, 2020 No. 498 "Some questions of provision of insurance payments in case or the death of health workers in connection with infection with sharp respiratory disease of COVID-19, of SARS-CoV-2 caused by coronavirus" (The Official Bulletin of Ukraine, 2020, No. 51, the Art. 1598, No. 101, the Art. 3272), changes which are applied.

Changes which are made to the resolution of the Cabinet of Ministers of Ukraine of June 17, 2020 No. 498

1. According to the procedure of implementation of insurance payments in the case or the death of health workers in connection with infection with sharp respiratory disease of COVID-19, of SARS-CoV-2, caused by coronavirus and determinations of their sizes approved by the specified resolution:

1) in Item 2:

word in paragraph one "calendar year owing to" shall be replaced with words "years from the date of";

paragraphs two - the fourth after words of "calendar year" to add with words "in which the group of disability is established";

2) in Item 3:

the paragraph one after words of "calendar year" to add with words "in which the health worker died";

add Item after the paragraph of the fourth with paragraphs of the following content:

"the unmarried full age son, the daughter who are recognized as persons with disability since the childhood;

unmarried full age son, the daughter who study in day form of education and not reached 23-year age;".

With respect thereto to consider paragraphs of the fifth and sixth respectively paragraphs the seventh and eighth;

Items 4 and 7 to state 3) in the following edition:

"4. Insurance payments, the stipulated in Item 2 presents of the Procedure (further - insurance payment), are appointed and paid by managements of executive directorate of Fund in areas, Kiev or their departments (further - bodies of Fund) within one month from the date of emergence of the right to such payments for the address of the health worker in the registered place of residence/stay.

The right to insurance payment comes from the date of acceptance by body of Fund of the statement and the documents determined by paragraphs the fifth - the eleventh this Item.

For purpose of insurance payment the health worker gives to body of Fund:

the statement for purpose of payment with indication of the personal account opened in bank institution for money transfer and information on non receipt of payment in other body of Fund for the form approved by executive directorate of Fund;

the passport copy of the citizen of Ukraine (for foreigners and stateless persons which live in Ukraine - the copy of the passport document of the foreigner and/or the certificate on permanent residence) with presentation of the original (the copy is certified by the signature of the employee of body of Fund) or, in case of technical capability, passports of the citizen of Ukraine in the form of reflection electronically information which contains in the passport of the citizen of Ukraine in the form of card, and also information, documents (data, data) about registration number of accounting card of the taxpayer (in the presence), the residence (in the presence), the electronic services represented by means of the Single state web portal (further - the Action Portal);

the copy of registration number of accounting card of the taxpayer (for persons who because of the religious beliefs refuse adoption of registration number of accounting card of the taxpayer and reported about it to the relevant monitoring body, - the copy of the page of the passport of the citizen of Ukraine with mark about the right to make any payments on series and/or passport number) with presentation of the original (the copy is certified by the signature of the employee of body of Fund) or its electronic copy if the receiver of information has technical capability to carry out by information and telecommunication means with use of means of technical and cryptographic information security according to requirements of the legislation concerning information security check of compliance of registration data of physical person to data of the State register of physical persons taxpayers;

the certificate of results of determination of extent of loss of professional working capacity as a percentage, needs in provision of the medical and public assistance which are issued by medico-social commission of experts;

copy of the act of investigation (special investigation) of accident, acute occupational disease (poisoning), accident in the established form;

the copy of the service record which is certified the signature by the employee of body Fonda, with presentation of the original or the copy of the statement from it certified by the insurer or data on labor activity from the register of insured persons of the State register of obligatory national social insurance;

the copy of the civil agreement (if the health worker worked at conditions of such agreement), which is certified by the insurer or which is certified the signature by the employee of body Fonda in case of presentation of the original;

the judgment about factual determination of occupational disease (in the presence of the judgment on this matter).

The statement, documents and copies of documents move in paper or, in case of technical capability, electronically with imposing of the digital signature based on the qualified digital signature certificate according to requirements of the legislation.

In the presence of electronic information exchange of Fund with state bodies, local government bodies, the companies, organizations or the organizations in which ownership there is information necessary for purpose of insurance payment such information by the health worker does not move.";
