1. Approve:

1) the list of the data necessary for implementation of cameral control according to appendix 1 to this order;

2) rules of submission of the list of the data necessary for implementation of cameral control by customs authorities, authorized body of technical regulation according to appendix 2 to this order.

2. Declare invalid:

1) the order of the Minister of national economy of the Republic of Kazakhstan of June 5, 2015 No. 421 "About approval of the List of the data necessary for implementation of cameral control, and also Rules of their representation by authorized bodies in the field of customs affairs and in the field of technical regulation, bodies for confirmation of conformity and test laboratories (centers)" (it is registered in the Register of state registration of regulatory legal acts at No. 11904, it is published on September 14, 2015 in the Information system of law of Ad_let);

2) the order of the Minister of national economy of the Republic of Kazakhstan of November 11, 2016 No. 474 "About introduction of amendments to the order of the Minister of national economy of the Republic of Kazakhstan of June 5, 2015 No. 421 "About approval of the List of the data necessary for implementation of cameral control, and also Rules of their representation by authorized bodies in the field of customs affairs and in the field of technical regulation, bodies for confirmation of conformity and test laboratories (centers) (it is registered in the Register of state registration of regulatory legal acts at No. 14645, it is published on January 19, 2017 in Reference control bank of regulatory legal acts of the Republic of Kazakhstan).

3. To provide to committee of sanitary and epidemiologic control of the Ministry of Health of the Republic of Kazakhstan in the procedure established by the legislation of the Republic of Kazakhstan:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) placement of this order on Internet resource of the Ministry of Health of the Republic of Kazakhstan;

3) within ten working days after state registration of this order in the Ministry of Justice of the Republic of Kazakhstan submission to Legal department of the Ministry of Health of the Republic of Kazakhstan of data on execution of the actions provided by subitems 1) and 2) of this Item.

4. To impose control of execution of this order on the supervising vice-Minister of Health of the Republic of Kazakhstan.

5. This order becomes effective after ten calendar days after day of its first official publication.

List of the data necessary for implementation of cameral control

1. The list of the data necessary for implementation of cameral control provided by authorized body in the field of customs affairs:

1) country of departure;

2) registration number of the declaration on goods (further - DT);

3) department of state revenues of declaring;

4) country of goods' origin;

5) sender and (or) exporter;

6) surname, name, middle name (in case of its availability) and (or) the name of the participant of foreign economic activity (further - UVED);

7) individual identification number (further - IIN) and (or) business identification number (further - the BIN) UVED;

8) UVED, legal address;

9) commodity code according to the commodity nomenclature of foreign economic activity of the Eurasian Economic Union (further - the CN FEA EEU);

10) description of goods;

11) the description of goods from DT;

12) goods weight, net (kilogram);

13) quantity in additional units of measure (further - DEI);

14) mark about conditional release of goods.

2. The list of the data necessary for implementation of cameral control provided by authorized body in the field of technical regulation:

1) surname, name, middle name (in case of its availability) and (or) the name, the address, the BIN and (or) the applicant's IIN;

2) name of the manufacturer (producer);

3) code (codes) according to the commodity nomenclature of foreign economic activity of the Eurasian Economic Union (CN FEA EEU);

4) quantitative data on goods;

5) information on regulatory legal acts and regulating documents on which compliance to requirements confirmation of conformity was carried out;

6) information on the documents submitted by the applicant to body for confirmation of conformity as the proof of compliance of products to requirements of regulatory legal acts, information on the conducted researches (testing) and measurements (date, number of the test report, the name and number of the certificate of accreditation of the test laboratory which issued the test report, date, number of the foreign certificate, the name of body it issued);

7) date and reason of suspension, renewal or cancellation of the certificate of conformity (declaration of conformity);

8) date, renewal term of action of the certificate of conformity (declaration of conformity) and basis for its prolongation;

9) data on appendix (appendices) to the certificate of conformity (declaration of conformity);

10) the name, the address, number and date of issue of the certificate of accreditation of the body for confirmation of conformity which issued (registered) the certificate of conformity (declaration of conformity);

11) surname, name, middle name (in case of its availability) the head of the certification body which issued (registered) the certificate of conformity (declaration of conformity);

12) surname, name, middle name (in case of its availability) the expert-auditor (expert);

13) effective period, registration certificate number of compliance (declaration of conformity), accounting number of the form on which the certificate of conformity (declaration of conformity) is drawn up.
