1. Establish form of the radiation and hygienic passport of the user of source of ionizing radiation according to appendix.
2. Approve the Instruction about procedure for maintaining and use of the radiation and hygienic passport of the user of source of ionizing radiation it (is applied).
3. Recognize invalid:
the resolution of Council of Ministers of the Republic of Belarus of June 24, 2006 No. 797 "About the radiation and hygienic passport of the user of sources of ionizing radiation, procedure for its maintaining and use and recognition No. 391 which voided resolutions of Council of Ministers of the Republic of Belarus of March 23, 1999";
subitem 1.19 of Item 1 of the resolution of Council of Ministers of the Republic of Belarus of December 30, 2010 No. 1910 "About modification and amendments in some resolutions of Council of Ministers of the Republic of Belarus and recognition voided some orders of the Government of the Republic of Belarus concerning licensing".
4. This resolution becomes effective after its official publication.
