About approval of the Instruction for filling of booking and accounting and route sheets
According to the Law of the Republic of Uzbekistan "About consumer protection" and the resolution of the Cabinet of Ministers of the Republic of Uzbekistan of July 11, 1998 "About creation of the Uzbek agency of road and river transport" for the purpose of establishment of procedure in filling of booking and accounting and route sheets with carriers, and also in accounting, I order to No. 296:
1. Approve the enclosed "Instruction on filling of booking and accounting and route sheets".
2. Costitute this Instruction after ten days from the moment of its state registration in the Ministry of Justice of the Republic of Uzbekistan.

This Instruction is developed according to the resolution of the Cabinet of Ministers of the Republic of Uzbekistan of July 11, 1998 No. 296 "About creation of the Uzbek agency of road and river transport" and the Law "About Consumer Protection" of April 26, 1996 and determines procedure for filling of booking and accounting and route sheets.
This Instruction extends to legal entities, irrespective of the patterns of ownership performing regular city, suburban, long-distance, international carriages of passengers road transport.
1. In this Instruction the following basic concepts are applied:
the bus - the vehicle intended for public conveyance and having more than 8 places for sitting apart from driver's seat;
city transportations - the transportations performed within borders of the city or city settlement;
linear taxi - the vehicle rendering services in transportation in requests of passengers for a fee according to the indication of taximeter (counter);
route - the established travel line of vehicles between certain Items;
the share taxi - the bus or the car intended for public conveyance on regular routes with the organization of stops upon the demand of passengers;
long-distance transportations - the transportations performed outside borders of the city or city settlement on distance more than fifty kilometers, and also the transportations performed in the territory of two and more areas of the Republic of Uzbekistan irrespective of route extent;
international carriages - the transportations performed outside frontier or because of frontier of the Republic of Uzbekistan irrespective of route extent;
zero runs - run of railway vehicles from the company to Item of the beginning of work on the set route, or from close-up Item along the set route to the place of dislocation of the vehicle;
turnover run - run of railway vehicles along the set route from initial to terminal point and back in starting point where movement was begun;
the passenger - the physical person using services of carrier according to the signed transportation agreement (which is transported for a fee or freely by carrier);
carrier - the legal entity who is owning the vehicle on the property right or other corporeal rights, rendering on commercial basis services in public conveyance, baggage and having on it the license;
suburban transportations - the transportations performed outside borders of the city or city settlement on distance to fifty kilometers inclusive;
the schedule of movement - the schedule (table) containing data on time, the place and the sequence of accomplishment of run;
run - movement of the vehicle from the point of departure to destination.
2. Route sheets and forms of job specifications are the main documents characterizing work of vehicles of carrying-out public conveyance including linear taxi. For buses and share taxis also "Booking count sheets".
3. Forms traveling (all types), booking count sheets and forms of job specifications of hour vehicles including linear taxi, are stored on an equal basis with other forms of the strict reporting and in process of requirement are issued by accounts department according to requirements to person, responsible for operation of motor transport (the head of department of operation, the senior dispatcher and employees of department of operation) with indication of sequence numbers.
Responsible advance holders constitute and hand over the monthly report on expense of forms traveling, booking count sheets and job specifications in accounts department.
4. Issue of route sheets to drivers (including to drivers of linear taxi) is made by duty dispatchers, and booking count sheets and job specifications on hour buses are issued by duty cashiers in case of presentation of the route sheets resolving departure on route.
5. Issued to the driver traveling and booking count sheets shall have surely sequence number on this autocompany (since January 1 every year, date of issue, stamps of the autocompany which possesses this vehicle).
6. Responsibility for the correct filling of details of traveling documents is born by the chiefs of departments of operation, the senior and duty dispatchers and persons who are responsible for registration of release of vehicles to the line and their return to garage.
7. Corrections and blots in traveling, booking count sheets are not allowed.
8. The driver shall watch personally correctness of all records in traveling, the booking count sheets made along the line in case of arrivals on intermediate bus stations (bus stations), and on terminal stopping points.
9. Booking count sheets consist of two parts: the booking count sheet (to cut edge) and check sheet (after cut edge). Filling of both parts is obligatory.
10. Duty dispatchers, ticket clerks, drivers (conductors), linear dispatchers, cashiers of bus stations (bus stations) and intermediate autocash desks, and also cashiers on acceptance of revenue participate in filling of booking count sheets.
11. By the booking count sheet it is determined: exact expense of tickets on series and numbers, revenue size for the sold tickets, the number of the performed flights, including according to the plan, actually, with violation of the schedule (regularity indicator), and also general results on the number of the transported passengers and the carried-out passenger-kilometers.
12. Advance holders (the driver, the conductor) on ticket sales shall show prior to work to the duty dispatcher of bus station or bus station the booking count sheet together with tickets for check of correctness of filling in the booking count sheet of series and numbers of tickets for sale.
13. The head of department of operation of the autocompany bears responsibility for correctness of maintaining and filling of booking count sheets and daily checks their compliance of this Instruction.
14. Filling of details traveling and booking count sheets of all types applied in passenger motor transport including linear taxi, it is necessary to make according to this Instruction.
15. Filling of forms of job specifications of the buses working on hour contractual rates is made by the duty dispatcher of the autocompany and the ranking officer of the customer.
16. Details of route sheets (appendix No. 1): the name of the owner of the vehicle (stamp), month, year, the state and garage number of the car, surname, name, the driver's middle name, number of the car driver license, employee ID, class is filled in prior to the beginning of month in department of operation.
17. On the front face of the route sheet the dispatcher in the Section "Plan target" puts down the schedule (time of departure and return), working hours on the line in day, in month, revenue in day and in month, utilization rate of paid run.
18. On arrival for work the driver shall note receipt time for work at the dispatcher (inner side of the route sheet of p. 2), to show the certificate on the driving right, to receive at it the route sheet, then the driver shall have medical examination for receipt of permission of the doctor to departure for health reasons that makes sure the signature of the doctor of the Section "Mark of the Doctor" on the inner side of the route sheet.
19. In the Section "Watch of Motor Transportation Affinity in Items" on the front face of the route sheet the dispatcher opposite to date, specifies marks about the direction of the vehicle in certain Item, in the columns "about arrival" and "about departure" marks the dispatcher of Item where watch was carried out become.
20. The chief or the mechanic of column on the front face of the route sheet in the Section "The Vehicle, Taximeter and Seals Are Operational" appends the signature in the column "I Resolve Departure", and in case of return the signature in the column "Accepted the Vehicle, Checked Indications of Speedometer".
21. Having received the visa of the chief or mechanic of column, the driver appends the signature in the Section "The Vehicle, Taximeter and Seals Are Operational" in the column "Accepted the Vehicle", and in case of return appends the signature in the same Section in the column "Handed Over the Vehicle in Column".
22. After preparation of the vehicle for departure the driver shall receive fuel. The dispatcher in the Section "Signature of the Dispatcher" on the inner side of the route sheet (p. 2) appends the signature. After that the refueler issues fuel and in the Section "Fuel" writes down amount of the issued fuel and appends the signature.
23. Here on the inner side of the route sheet the relief engineer in the Section "Actual Time" notes stamp hours the actual time of departure, the indication of speedometer and the indication of taximeter (run general, paid, cash desk of landing). The relief engineer writes down the same indicators of taximeter in case of return of the vehicle from the line.
24. In the Section "Fuel" the relief engineer puts down fuel availability in case of departure and appends the signature, and in case of arrival notes fuel remaining balance in the vehicle, appends the signature.
On the inner side of the route sheet (p. 3) by dates until the end of month is made the same filling, as on the second page of the route sheet.
25. On the same page in the Section "Record of Country Trips" the mark is made:
the duty dispatcher - record time who permitted, from where, where, in one or both ends, indications of speedometer, the indication of taximeter, general run, paid run, cash desk, landings;
the relief engineer - about technical condition of the vehicle.
26. In the Section "Different Marks in the Line" necessary records during the operation of the vehicle on the line (the note of monitoring bodies, discontent of passengers) are made.
27. On the fourth page of the route sheet marks about the made arrivals, repairs and premature return of the vehicle to garage, the reasons of late check-out are made.
28. In the Section "Planned Maintenance" are put down by technical service planned TO-1, TO-2 and are confirmed by the signature of the head of the technical engineering department, and in the Section "Replacement of Taximeter" the mark about its replacement is made.
29. Processing of the route sheet is made by the dispatcher of the park and employees of service of operation.
30. On the inner side of the route sheet the dispatcher daily sums up the results of operation of the vehicle of taxi. In the line "Result" results of work on indications of speedometer and taximeter (general run, paid run, cash desk, landings) are daily put down, these results as difference of indications are determined in case of return of the vehicle of taxi from the line and in case of departure from garage. In the absence of taximeter the paid run is determined by multiplication of general run of the vehicle of taxi by planned coefficient of paid run; the number of landings is determined by reporting data of last months.
31. The column "Revenue" is filled in with the dispatcher based on data on paid run and plus the number of the landings increased by the established rate of one paid kilometer.
32. In the Section "Working hours" in the column "Hours in the Job Specification" hours are determined as difference between time of return of the vehicle of taxi to garage and time of departure from garage minus time for lunch.
"Hours on intra replaceable idle times" are determined by data of the Section "Marks about the Made Arrivals, Repairs and Premature Return of the Vehicle of Taxi to Garage".
"Night hours" - hours from 22-00 till 6-00 o'clock.
Hours for holidays are put down in the column "Holidays".
At the end of the month the dispatcher sums up the results in month and puts down result on the third page of the route sheet in the line "Total in Month".
33. The processed route sheets upon termination of month are transferred to accounts department on receipt in the terms established by the order on the company.
34. Details of route sheets (appendix No. 2): the name of the owner of the vehicle (stamp), number of the route sheet, month, year, first name, middle initial, last name the driver, employee ID, number of the certificate on the right of management, the brand, the state number and series, garage number of the bus are filled prior to the beginning of month in department of operation.
35. The day before or before departure of the driver to the line the duty dispatcher shall in the corresponding date of departure on the front face of the route sheet fill gr. 2 "Brand of the vehicle", gr. 3 "The state number of the vehicle", gr. 4 "Departure time according to the schedule", gr. 6 "Number of route", gr. 7 "Name of route or customer", gr. 8 "Number of the schedule", gr. 14 "Return time according to the schedule".
36. Based on route documentation, the traffic flow plan of buses, and also operating conditions of vehicles the duty dispatcher shall fill in the route sheet (gr.
18) "Runs according to the plan", (gr. 23) "Number of the booking count sheet", (gr. 24) "Revenue according to the plan".
37. Time of the actual departure (gr. 5) and the actual return of the vehicle from the line (gr. 15) it is put down by the relief engineer in case of release and acceptance of the vehicle from the line with the help stamp hours.
38. On the front face of the route sheet - permission to release from garage of the vehicle which is in technically serviceable condition, delivery to his driver and acceptance from the driver with indication of the actual time of acceptance (gr. 10, gr. 16), and also indications of speedometer on the vehicle in case of departure and return to garage (gr. 12, 13) make sure the signature of the relief engineer or the replacing his face.
38(1). Before departure of the driver to the line on the front face in gr. The 9th health worker the signature the state of health of the driver and possibility of its admission to control of the vehicle makes sure.
39. In case of acceptance of the vehicle by the driver he undersigns for garage in gr. 11th front face of the route sheet.
The second offer of Item 39 of paragraph 5 is excluded according to the Order of the chief of the Uzbek agency of road and river transport of the Republic of Uzbekistan of 15.07.2013 No. 8

40. Arrival time of the vehicle to Item of the beginning of work is put down on check sheet of the booking count sheet put down with the help stamp hours:
a) on city bus routes and routes of the share taxi linear dispatchers of starting and terminal points;
b) on suburban and long-distance routes dispatchers of bus stations (bus stations), autocash desks.
41. Record about fuel remaining balance in vehicle tank for the end of day and the accruing result (gr. 36 and 37), actual flow of fuel (gr. 38), including on route, according to the job specification and watch (gr. 39 and 40), are determined by person in charge for fuels and lubricants. Fuel quantity, returned in fuel station in cases of intertrip idle times, statements on repair, etc. (gr. 41) it is signed by the operator of fuel station (gr. 42).
42. The amount of the issued fuel is signed or confirmed by personal stamp of persons which are carrying out this issue (gr. 34, 35).
43. Marks about the reasons of delay in departure from garage are made by the relief engineer or other representative on that person in Section 1 of the route sheet (p. 4) "Marks about delay of the vehicle on exit from garage", and in Section 2 of the route sheet (p. 4) "Arrivals in garage and idle times of the vehicle on the line" are noted arrivals in garage during the working day.
44. Marks about duration of idle time of the vehicle in garage for technical reasons are done by the relief engineer in Section 2 of the route sheet (p. 4) "Arrivals in garage and idle times of the vehicle on the line", and on the line are done by the linear dispatcher of control office or initial bus station, specifying at the same time change, duration and the reason of idle time in the same Section.
All idle times of the vehicle caused by defect of the vehicle, replacement of tires, damage of the road and other reasons are specified.
45. When switching the vehicle to other route by the linear dispatcher marks in the Section 3 "Data on Switching of the Vehicle to Other Route" in the fourth page of the route sheet are made. At the same time the dispatcher warns about features of the route of movement and safety control of movement.
46. Data on check by competent authorities at passengers, about violations by the driver of service regulations of the vehicle on the line, registers in lines of availability of tickets in the Section 4 "Notes" on the fourth page of the route sheet.
47. In all cases, by provision of the vehicle by request, to the driver the job specification of the established form is issued (appendix No. 3). The name of the customer is entered in the column 7 of the front face of the route sheet, and the duty dispatcher puts down number of the job specification in gr. 23 "Number of the booking count sheet" of p. 3.
Upon termination of servicing "Job specification" shall be filled and signed person in charge for use of the vehicle with availability of stamp of the customer, and "Job specification" shall sign physical person.
48. Processing of the route sheet is made by the dispatcher and employees of service of operation responsible for operation of the vehicle. Prior to processing of the route sheet careful inspection of all records made by the linear dispatcher, the cashier or the dispatcher of bus station (bus station), drivers in the booking count sheet shall be carried out.
Based on entries in booking count sheets results of operation of the vehicle, the driver in day or change which are entered in the corresponding columns of the route sheet are established.
Data from the front face from gr. 1, 2, 3, 6, 7, 8 are transferred in gr. 1, 2, 3, 4, 5, 6, on the second page of the route sheet.
49. In gr. The 7th inner side of the route sheet "Mark about Switching to Other Route" data from Section 3 on the fourth page of the route sheet "Data on Switching of the Vehicle to Other Route" are entered.
50. Determination of specifications by results of work is made as follows (the inner side of the route sheet):
a) hours in the job specification (the second page of the route sheet) (gr. 8) are determined as difference between time of return of the vehicle to garage and time of departure from garage, minus time for lunch and rest established to the driver according to the schedule;
b) working hours on the line (gr. 9) shall constitute the amount of working hours of the vehicle on route (gr. 10) by the order (gr. 12); reserve; persons on duty (gr. 13);
c) hours on route (gr. 10) including according to the schedule (gr. 11) are determined by work number of hours only on route from the moment of arrival of the vehicle in Item of the beginning of work and prior to its congress from terminal point in garage. At this time does not enter: operating time by request, time of unproductive idle times for lines, arrivals in garage during the working day and lunch breaks, unjustified time.
51. Vehicle operating time by the order (gr. 12) it is determined by the job specification which is drawn up by the organization and calculated:
a) during the whole-day work of the driver by request - from the moment of departure from garage until return to garage with deduction of time for the driver's lunch;
b) in case of removal of the vehicle from route without its return to route - from the moment of removal of the vehicle from route before its arrival in garage;
c) in case of temporary removal from route with its return to route - from the moment of the congress from route to vehicle time of engagement in the schedule. About temporary removal from route and inclusion it in work the linear dispatcher makes corresponding entry in the booking count sheet.
52. In the column 13 "Reserve", "Persons on duty" - time spent for watch or idle time in reserve is put down.
53. In column 14 unjustified time is put down.
54. Idle time on the line (gr. 15) it is determined by summing of idle time on the line by the technical and operational reasons of records of linear dispatchers, chiefs of bus stations (bus stations).
55. Night time during the work of the driver on the line (including by request) is considered from 22-00 to 6-00 o'clock o'clock in the morning (gr. 16).
Festive time is calculated from 00 o'clock the come days of holiday to 24 o'clock in the morning (gr. 16).
56. In gr. 17 "Preparatory and final time" for convenience of accounting in department of operation is put down on 0,3 of hour on departure and arrival.
57. In columns 19, 20 (p. 2) "The actual number of runs" and "The number of the flights performed according to the schedule" are put down based on data of the linear dispatcher on the booking count sheet.
Hours on repair (gr. 21) are put down by the dispatcher based on data of technical service and the faces responsible for repair confirmed with the signature (gr. 22).
58. columns 25, of 26, of 27, of 28, of 29, 30 "The actual revenue" are filled in based on these booking count sheets which already underwent processing at duty dispatchers and the cashier.
59. Calculation of amounts of transportations in passengers and passenger-kilometers is made according to Regulations on procedure for determination of passenger turnover and the number of the passengers transported by automobile and electric common carrier on city, suburban, long-distance, international routes and hourly transportations in the Republic of Uzbekistan, to No. 273 approved by the resolution of the Cabinet of Ministers of the Republic of Uzbekistan of September 17, 2012 (Collection of the legislation of the Republic of Uzbekistan, 2012, No. 38, the Art. 437).
60. Vehicle run (gr. The duty dispatcher determines 31) by indications of speedometer and controls by multiplication of number of runs to distance of one run, plus zero run on this route (the run depending on the number of runs taking into account zero run shall be developed by department of operation of each route and is reduced in the subsidiary tables necessary for convenience and acceleration of processing of route sheets).
Write-off of fuel is made on run on speedometer if it is confirmed with the performed flights taking into account zero run. If the settlement run on the performed flights taking into account zero run is less, than the actual run on speedometer, that write-off of fuel is made on run on the performed flights taking into account zero run.
61. Run with passengers (gr. 32, 33) it is determined by multiplication of general run by planned utilization rate of run.
62. The petrol consumption on regulation is determined daily by general run of the vehicle according to the indication of speedometer (in day or change) and to the approved fuel regulations.
Amount of actually spent fuel (gr. 38, 39, 40) it will be equal to amount of fuel in the vehicle at the time of departure from the park minus remaining balance with which the vehicle returned to the park.
63. Route sheets upon termination of month shall be checked and processed by the senior dispatcher, the economist and the head of department of operation by means of the analysis of accomplishment of the plan of transportations, revenue, discipline of the driver, the offer to them.
64. When processing route sheets the department of operation shall sum up the results the accruing procedure for 15, for 25, 30 days (or 31).
The processed route sheets upon termination of month are sewed in the folder according to the procedure of their numbers and transferred to accounts department on receipt in the terms established by the order on the autocompany.
65. Persons allowing the wrong issue of route sheets, their wrong filling and additions distorting the valid operation of the vehicle are subject to accountability in accordance with the established procedure.
66. Columns of the route sheet are filled in:
a) duty dispatcher: on the front face of the route sheet - gr. 2, 3, 4, 6, 7, 8 and 14; on the inner side of the route sheet - gr. 2–24, 31, 32 and 33;
b) relief engineer gr. 5, 10, 12, 13, 15 and 16 front face of the route sheet;
c) the health worker - gr. 9th front face of the route sheet;
d) the driver - gr. 11th front face of the route sheet;
e) the cashier - gr. 25–30 inner sides of the route sheet;
e) the operator of fuel station - gr. 34, 35, 41 and 42 inner sides of the route sheet;
g) person in charge for fuels and lubricants – gr. 36-40 inner sides of the route sheet.
67. In filling and processing of booking count sheets (appendices No. 4, No. 5) participate duty dispatchers and ticket clerks, drivers (conductors), cashiers accepting revenue.
68. The following obligations are assigned to the specified workers:
a) the ticket clerk fills in Sections on the tickets issued to drivers or conductors with indication of type, advantages, series and numbers of the upper tickets. These data register both in the booking count sheet, and in the corresponding graphs of check sheet (appendices No. 4, No. 5). Besides, the ticket clerk makes records of date of issue of the booking count sheet, number of the vehicle, surname of the driver (conductor) and results of check of correctness of calculation of the actual revenue.
Total amount on the tickets issued to the drivers working at suburban and long-distance routes (during their work through bus stations (bus stations) and autocash desks), there shall be at least 70% of the total amount of revenue on this route taking into account sheets of bus station (bus station) and autocash desks (appendices No. 6, No. 7).
b) in case of issue to the driver (conductor) of the booking count sheet with tickets the ticket clerk issues it at the same time change in the amount determined by route and enters to it the amount of the booking count sheet after the list of issued tickets.
The subsequent issue of the booking count sheet is made by the cashier in case of acceptance of revenue for the sold tickets, at the same time instead of handed over one form of the booking count sheet is issued;
c) the duty dispatcher fills in check sheet of route of the vehicle time of the beginning and close-up, and also results of operation of the vehicle, that is the number of the transported passengers and executed passazhiro/kilometers;
d) drivers and conductors make in check sheet poreysovy records of the sold tickets with indication of series, numbers and the price of the upper tickets. Upon termination of work they write down total quantity of the realized tickets and revenue amount.
All pretrip records about the sold tickets shall be certified by stamp and the signature of workers of checkpoints;
e) linear controllers in case of control shall require from the driver the booking count sheet, make check of compliance of series and numbers of the tickets which are written down in the booking count sheet, availability of tickets at the conductor (driver), to do mark about check in the booking count sheet, putting down time of check and to ratify it;
g) cashiers of bus stations (bus stations) and intermediate autocash desks on the back of check sheet on each run write down the number of the tickets sold on each vehicle with indication of series, numbers of tickets and their price.
All marks shall be confirmed with the signature of the cashier of bus station (bus station) or intermediate autocash desk with appendix of stamp;
h) cashiers on acceptance of revenue in case of issue of the new booking count sheet instead of handed over upon termination of work shall write down in the column "Prior to Work" all data on the tickets which remained with the driver or the conductor after change with indication of numbers series of the upper tickets.
Record of these data by the driver, conductor or other person is forbidden.
In case of acceptance of the amounts of revenue and backs of zone tickets on the back of the booking count sheet write down data on the accepted cash revenue in the Section "It Is Accepted in Cash Desk".
69. When filling separate Sections of the booking count sheet it is necessary to be guided by the following instructions:
a) Section 1. Data of drivers, conductors, marks of bus stations (bus stations).
Along the line of the vehicle the conductor or the driver shall make for control of correctness of ticket sales by the conductor (driver) in "Check sheet" poreysovy record of numbers and series of the last tickets on certain Items of routes.
Checkpoints of marks are established for each route separately.
In case of work on route of the vehicle of several haulers checkpoints are established by superior organization.
For the purpose of saving of time the driver or the conductor can write down in the booking count sheet only the last three digits of six-digit numbers against each kind of tickets.
The reconciliation of correctness of record of numbers and series of tickets is performed: in transit competent authorities, on terminal and intermediate points - the person on duty on bus station (bus station).
Upon termination of work the driver or the conductor count results according to the Section 1 "Data of Drivers, Conductors, Marks of Bus Stations (Bus Stations)", write down data on the number of the sold tickets and the realized sums on each advantage and confirm these results with the signature.
In case of sale of zone tickets the driver after close-up puts backs of tickets on cut belts, counts the number of identical tickets on cut belts, counts the number of identical tickets on belts, enters these data in the corresponding columns of the Section 1 "Zone Tickets" and fastens them with the signature.
The number of the sold rolled tickets is determined by each advantage as difference between final and initial numbers of the corresponding series;
b) Section 2. Data of autocash desks (for long-distance routes).
This Section of the booking count sheet (appendix No. 4) is filled only with duty cashiers of intermediate bus stations (bus stations) and autocash desks. Data are filled in separately on even and odd runs.
At the same time the duty cashier of bus station (bus station) or intermediate autocash desk shall fill in all columns of the Section, namely: flight number, series and numbers of initial and final tickets, the price and the number of the sold tickets.
On advance sale of tickets the cashier shall note in check sheet the number of the sold tickets according to the sheet.
70. Processing of booking count sheets and determination of the executed amount of transportations in passazhiro/kilometers are made by the duty dispatcher based on data of Section 1 of check sheet.
