About approval of Rules of performing medical examinations of staff in military-medical (medical) divisions of Armed Forces of the Republic of Kazakhstan

According to the subitem 10) article 8 of the Code of the Republic of Kazakhstan of September 18, 2009 "About health of the people and health care system", PRIKAZYVAYU:

1. Approve the enclosed Rules of performing medical examinations of staff in military-medical (medical) divisions of Armed Forces of the Republic of Kazakhstan.

2. And arms of Armed Forces of the Republic of Kazakhstan in the procedure established by the legislation of the Republic of Kazakhstan to provide to the chief Tyla:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) placement of this order on Internet resource of the Ministry of Defence of the Republic of Kazakhstan after its first official publication;

3) the direction of data in Legal department of the Ministry of Defence of the Republic of Kazakhstan on execution of the actions provided by subitems 1) and 2) of this Item within ten calendar days from the date of state registration.

3. To impose control of execution of this order on the supervising deputy defense secretary of the Republic of Kazakhstan

4. Bring this order to officials in the part concerning them.

5. This order becomes effective after ten calendar days after day of the first official publication.

Rules of performing medical examinations of staff in military-medical (medical) divisions of Armed Forces of the Republic of Kazakhstan

Chapter 1. General provisions

1. Rules of performing medical examinations of staff in military-medical (medical) divisions of Armed Forces of the Republic of Kazakhstan (further - Rules) determine procedure and frequency of performing medical examinations of staff in military-medical (medical) divisions of Armed Forces of the Republic of Kazakhstan.

2. Medical examinations are performed for the purpose of ensuring the timely physical examination directed to forming and strengthening of health, identification and the prevention of spread of diseases, including professional, poisonings, accidents and also on safety of military service.

3. The medical examinations performed according to these rules include:

1) preliminary obligatory medical examinations of staff;

2) prereplaceable obligatory medical examinations of staff;

3) periodic obligatory and routine medical examinations of the military personnel;

4) dynamic observation of the state of health of the military personnel.

Chapter 2. Procedure for performing preliminary obligatory medical examinations of staff

4. Preliminary obligatory medical examinations are performed in case of receipt (appeal) on military services (work or study) for the purpose of clarification of suitability to accomplishment of obligations by profession or study, and also preventions of general, professional and non-proliferation of infectious and parasitic diseases.

5. Preliminary obligatory medical examinations arriving (called) on military service (study) are performed by medical examination according to the order of the Minister of Defence of the Republic of Kazakhstan of July 2, 2015 No. 373 "About approval of Rules of conducting military-medical examination and Regulations on bodies of military-medical examination in Armed Forces of the Republic of Kazakhstan" (it is registered in the Register of state registration of regulatory legal acts for No. 11846) (further - Rules of military-medical examination).

6. Preliminary obligatory medical examinations going to work are carried out to military units and organizations of Armed Forces of the Republic of Kazakhstan according to the order of the acting minister of national economy of the Republic of Kazakhstan of February 24, 2015 No. 128 "About approval of Rules of performing obligatory medical examinations" (it is registered in the Register of state registration of regulatory legal acts for No. 10634).

Chapter 3. Procedure for performing prereplaceable obligatory medical examinations of staff

7. Prereplaceable obligatory medical examinations of staff are performed by the doctor (the paramedic, the nurse) in specially equipped room (place).

8. Are exposed to prereplaceable obligatory medical examinations:

1) staff of guard, forces (changes) on duty - before zastupleniye in guard and on alert, active service

2) drivers - before departure in flight;

3) all military personnel decreasing in other military units, the newcomers in military unit and also who returned from leaves, business trips and medical institutions on recovery - in day of arrival in military unit, but to the direction them in divisions;

4) participants of sports meets - before competition;

5) workers of the dining room and the staff appointed in the job specification on the dining room - prior to the next change;

6) all military personnel of conscription military service - in bath before washing.

Besides, once a week in time established by the commander of military unit, medical (corporal) examination of the military personnel of conscription military service of divisions is performed.

9. The amount of prereplaceable obligatory medical examination (except for the medical examination of the military personnel of conscription military service performed in bath before washing and in division) includes:

1) poll about health, prior rest (dream), availability of claims to the state of health;

2) identification of variations in emotional condition (on appearance, behavior, communication form);

3) survey of open sites of integument;

4) calculation of pulse on beam artery within 30 seconds.

Measurement of body temperature, arterial pressure, and also carrying out additional methods of inspections is performed in the presence of the medical indication by results of the performed inspection.

Persons showing the claims to the state of health having variations of physiological indicators of arterial pressure, body temperature, signs of pustulous diseases of skin, the suppurated cuts, burns, grazes, displays of sharp diseases including infectious to service (work) go to military-medical organizations (division) for further inspection and treatment.

10. The amount of the medical (corporal) examination of the military personnel of conscription service performed by health workers of military unit (organization) in barracks arrangement (in medical division), and also in bath before washing includes: poll about health, availability of claims to the state of health, survey of integuments.

The health worker reports on the revealed bodily harms on the direct commander (chief). The military personnel having skin diseases in bath is allowed to washing in the last queue.

11. Results of medical examination are considered in register of medical examination in form according to appendix 1 to these rules.

Chapter 4. Procedure for performing periodic obligatory and routine medical examinations of the military personnel

Paragraph 1. Organization of performing periodic obligatory and routine medical examinations of the military personnel

12. Periodic obligatory and routine medical examinations of the military personnel include:

1) primary and periodic medical examination of the military personnel of conscription service, cadets, cadets;

2) routine medical examination of the military personnel passing military service under the contract on positions of lists of soldiers (sailors), sergeants (foremen) and officers;

3) medical examination of the staff leaving in long business trips (training) in foreign states;

4) medical examination of the staff occupied at works with radioactive materials, sources of ionizing radiation, components of rocket fuel and sources of electromagnetic fields.

13. Performing periodic obligatory and routine medical examinations of staff is organized by health service (department) of type, type of military forces of Armed Forces, troops of regional command, garrison.

For the organization of performing medical examination of staff the order of the commander-in-chief (commander) of types, types of military forces of Armed Forces, is issued by troops of regional command, the chief of garrison which is carried to commanders (chiefs) of military units (organizations), and separate divisions. The order determines terms and the venue:

1) fluorographic (radiological) inspection;

2) laboratory and tool inspections;

3) physical examination by doctors for each military unit (organization) and separate divisions;

14. Medical examination of staff is performed in out-patient conditions in medical aid station (medical company, platoon) of military unit (organization) and (or) military polyclinic (hospital, infirmary) according to daily routine established in military-medical division. In case of day lighting, for working hours one doctor no more than 40 people are examined.

15. By preparation and performing medical examination in the medical book of the serviceman the following data are generalized:

1) epikriz about current of the basic and associated diseases, availability and frequency of their aggravations in the expired year, and also about results of holding treatment-and-prophylactic actions;

2) aggregated data about number of cases and days trudopoter, the cases of hospitalization caused by aggravations of the basic and the accompanying chronic diseases;

3) conclusions about basic reason of dynamic observation of the serviceman (chronic disease, the postponed acute disease, availability of risk factors, work in the conditions of impact of adverse factors of military service);

4) conclusion about group of the state of health, group of physical training (contract servicemen), availability of risk factors and list of recommended treatment-and-prophylactic actions.

16. The health service (department) of type, type of military forces of Armed Forces, troops of regional command, garrison exercises control of completeness of scope, quality of performing medical examination and methodical help is given to doctors of military units (organizations) when performing medical examination and dynamic observation.

17. The health service of military unit (organization) provides completeness and quality of performing medical examination, dynamic observation, and also accomplishment of amount of the appointed treatment-and-prophylactic actions to the attached military personnel.

Paragraph 2. Procedure for performing primary and periodic medical examination of the military personnel of conscription service, cadets, cadets

18. Primary medical examination of the military personnel of conscription service, cadets, cadets of military educational institution (further - VUZ), is carried out within the first month of stay in military unit (organization).

The obligatory amount of primary medical examination includes:

1) fluorography (X-ray analysis) of bodies of thorax at persons which were not passing it in the last three months to appeal (receipt);

2) general blood tests, urine, microreaction;

3) anthropometry (growth, body weight, thorax circles), determination of index of body weight (further - IMT);

4) determination of visual acuity and hearing;

5) inspection by the therapist (general practice).

19. During inspection by the therapist (general practice) are carried out:

1) survey of integuments and hairy part of the head;

2) survey of visible mucous membranes: conjunctivas of eyes, oral cavities, pharynx, it is paid attention to dental health, bite, size and type of palatal almonds, voice function;

3) survey and palpation of area of thyroid gland;

4) palpation of peripheral lymph nodes: submaxillary, inguinal, axillary;

5) the research of bodies of blood circulation (survey, heart auskultation), determination of frequency, filling, rhythm of pulse, size of arterial pressure, in cases of identification of warm noise conduct researches in various provisions (standing, lying) and functional tests with the dosed physical activity;

6) research of respiratory organs (auskultation of lungs);

7) research of digestive organs (palpation of abdominal organs, areas of the right podreberye, liver, epigastralny area, spleen, right and left podvzdoshny areas, nadlobkovy area);

8) survey of genitals.

On medical indications additional methods of inspection, consultation of profile specialists doctors are appointed.

20. Periodic medical inspections of the military personnel of conscription service are performed once in six months in the amount determined in Item 18 of these rules.

21. Periodic medical examinations of cadets, cadets during training in HIGHER EDUCATION INSTITUTIONS are carried out annually in the first half of the year of academic year in the amount determined in Item 18 of these rules.

22. Upon termination of primary and periodic medical examination by the doctor of military unit (organization) or the therapist (general practice), taking into account results of objective inspection and laboratory diagnostic testings, complex assessment of the state of health with determination of the following of "groups of health" is carried out:

1) IA group - healthy without risk factors: persons who are not showing any claims and at which in the anamnesis and during survey chronic diseases, risk factors or violations of functions of separate bodies and systems are not revealed;

2) IB group - healthy with risk factors: persons with the revealed risk factors, "boundary" conditions needing observation and preventive intervention (persons at whom the insignificant variations from the established regulation borders in sizes of arterial pressure and other physiological characteristics which are not influencing functional activities of organism are revealed);

3) the II group - almost healthy: persons having in the anamnesis acute and (or) chronic disease without aggravations within the last two years;

4) III group - patients: persons having chronic diseases in condition of subcompensation (decompensation) with violations of functionality of organism.

Persons belonging to the II group are subject to dynamic observation and improvement by the doctor of military unit (organization) with involvement of profile specialists of military polyclinics (hospitals, infirmaries).

The faces carried in III group are subject to the direction on out-patient (stationary) treatment (rehabilitation) with the subsequent dynamic observation. Depending on results of treatment (rehabilitation) by the chief of health service (doctor) of military unit (organization) feasibility of the direction of the serviceman on medical examination for determination of category of the validity to military service (training) is determined.

23. Results of the performed medical examination and content of treatment-and-prophylactic actions are entered in the medical book of the serviceman of conscription service, the cadet, cadet in form according to appendix 2 to these rules.

Paragraph 3. Procedure for performing routine medical examination of the military personnel passing military service under the contract on positions of lists of soldiers (sailors), sergeants (foremen) and officers

24. The military personnel passing military service under the contract on positions of lists of soldiers (sailors), sergeants (foremen) and officers (further - contract servicemen) has routine medical examination once a year (further - PMO).

25. I will eat around PMO for contract servicemen aged up to 40 years:

1) fluorography (X-ray analysis) of bodies of thorax;

2) anthropometry (determination of body weight, growth, IMT, circle of thorax, stomach);

3) general blood tests, urine, microreaction;

4) electrocardiographical research in case of the first passing of inspection, further at the age of 35 years is also more senior once a year;

5) determination of visual acuity and hearing;

6) inspection by the general practitioner (therapist) according to the procedure determined by Item 19 of these rules;

7) on medical indications additional methods examinations, consultations of specialists doctors are held.

26. I will eat around PMO for contract servicemen at the age of 40 years and is more senior:

1) fluorography (X-ray analysis) of bodies of thorax;

2) anthropometry (determination of body weight, growth, IMT, circle of thorax, stomach);

3) general blood tests, urine, microreaction;

4) research of sugar of blood, blood serum cholesterol;

5) electrocardiographical research;

6) measurement of intraocular pressure (to destination ophthalmologist);

7) survey of specialists doctors (the surgeon, the therapist, the ophthalmologist, the stomatologist, the neuropathologist, the otorhinolaryngologist, the gynecologist - for the female military personnel).

In the presence of medical indications additional methods of inspections, consultations of specialists doctors are held.

27. Complex assessment of the state of health of contract servicemen, their distribution on groups of health, need of dynamic observation, and also feasibility of the direction of the serviceman on medical examination for determination of category of the validity to military service by results of PMO is performed according to Item 22 of these rules.

When implementing complex assessment of the state of health of contract servicemen results of the screening inspections performed by subjects of health care according to the order of the acting minister of health care of the Republic of Kazakhstan of November 10, 2009 No. 685 "About establishment of target groups of persons which are subject to routine medical examinations and also rules and frequency of carrying out these surveys" are used (it is registered in the Register of state registration of regulatory legal acts for No. 5918).

28. In case of registration of final results of PMO by the chief of health service (doctor) of military unit (organization) for contract servicemen age groupes of physical training according to the order of the Minister of Defence of the Republic of Kazakhstan of August 10, 2017 No. 438 "About approval of Rules of physical training in Armed Forces of the Republic of Kazakhstan" are determined (it is registered in the Register of state registration of regulatory legal acts for No. 15729).
