1. The insurance company (except for the medical insurance company performing activities only in the sphere of compulsory medical insurance) (further - insurance company) shall give in the Bank of Russia the following information concerning each credit institution to which according to Item 1.5-1 of article 7 of the Federal law "About Counteraction of Legalization (Washing) of Income Gained in the Criminal Way and to Terrorism Financing" carrying out identification or the simplified identification of the client - physical person, and also identification of the representative of the client, the beneficiary and the beneficial owner is entrusted (further - identification):

the complete and reduced (in the presence) trade names of credit institution (its branch), registration number of credit institution (the sequence number of its branch which is put down through fraction after registration number of credit institution) according to the Book of state registration of credit institutions;

agreement number based on which carrying out identification is entrusted (further - the contract for carrying out identification), and date of its conclusion.

2. The insurance company shall give in the Bank of Russia information, stipulated in Item 1 this Instruction, within 5 working days after date of the conclusion of the contract for carrying out identification.

3. The insurance company, in case of agreement cancelation on carrying out identification, shall give in the Bank of Russia information, stipulated in Item 1 this Instruction, and information on date of agreement cancelation on carrying out identification within 5 working days after date of its termination.

4. Information provided by Items 1 and 3 of this Instruction is submitted insurance company in the Bank of Russia in electronic form, signed by the strengthened qualified digital signature of the head of insurance company, according to requirements of the Instruction of the Bank of Russia of November 3, 2017 to No. 4600-U "About order of interaction of the Bank of Russia with credit institutions, not credit financial credit institutions and other participants of information exchange when using of information resources of the Bank of Russia, including private office by them", No. 49605 registered by the Ministry of Justice of the Russian Federation on January 11, 2018.

5. This Instruction becomes effective after 10 days after day of its official publication.

6. The insurance company shall give in the Bank of Russia information on credit institutions to which carrying out identification according to the contracts for carrying out identification existing on the date of entry into force of this Instruction, according to the procedure, established by this Instruction is entrusted, within 30 calendar days after day of entry into force of this Instruction.
