According to Article part three 2, to Articles 5 - 7 Laws of Ukraine "About the basic principles of the state supervision (control) in the sphere of economic activity", Article parts two 6, parts seven of article 19 of the Law of Ukraine "About licensing of types of economic activity", to Item 13 of article 8 of the Law of Ukraine "About state regulation of the security market in Ukraine" the National commission on securities and the stock market RESHILA:

1. Approve the unified form of the Statement which is drawn up by results of holding planned (unplanned) action of the state supervision (control) for observance by the subject of managing of requirements of licensed conditions in the sphere of implementation of professional activity in the stock market (security market) which is applied.

2. To provide to department of conducting inspections of professional activity:

submission of this decision on state registration in the Ministry of Justice of Ukraine;

promulgation of this decision on the official website of the National commission on securities and the stock market.

3. This decision becomes effective from the date of its official publication.

4. Control over the implementation of this decision to assign to the member of the National commission on securities and the stock market I. Nazarchuk.
