According to Items 298. 1, 298.7 Articles 298 and to Item 299.10 of Article 299 of Chapter 1 of the Section XIV of the Tax code of Ukraine, the subitem 5 of item 4 of the Regulations on the Ministry of Finance of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of August 20, 2014 No. 375, ORDER:

1. Approve such which are applied:

the application form about application of simplified taxation system;

form of calculation of the income for the previous calendar year preceding year of transition to simplified taxation system;

request form about receipt of the statement from the register of payers of the single tax;

statement form from the register of payers of the single tax.

2. Declare invalid the order of the Ministry of Finance of Ukraine of December 20, 2011 No. 1675 "About approval of form of the certificate of the payer of the single tax and procedure for issue of the certificate, form and procedure for submission of the statement for application of simplified taxation system and form of calculation of the income for the previous calendar year", registered in the Ministry of Justice of Ukraine on December 27, 2011 for No. 1536/20274.

3. In accordance with the established procedure to provide to department of tax policy:

submission of this order on state registration in the Ministry of Justice of Ukraine;

promulgation of this order.

4. This order becomes effective from the date of its official publication.

5. I reserve control of execution of this order and I assign to the acting as the Chairman of the Public fiscal service of Ukraine Vlasov A. S.
