According to the paragraph to the fifth Item 3 of the resolution of the Cabinet of Ministers of Ukraine of March 25, 2015 No. 302 "About approval of sample of the form, technical description and the Procedure for registration, issue, exchange, transfer, withdrawal, return to the state, recognitions invalid and destructions of the passport of the citizen of Ukraine", to the resolution of the Cabinet of Ministers of Ukraine of April 3, 2019 No. 398 "About modification of Item 3 of the resolution of the Cabinet of Ministers of Ukraine of March 25, 2015 No. 302" PRIKAZYVAYU:

1. Approve the Temporary procedure for registration and issue of the passport of the citizen of Ukraine which is applied.

2. To management of interaction with the Public migratory service of Ukraine (Kovalevskaya I. V.) provide submission of this order on state registration in the Ministry of Justice of Ukraine in accordance with the established procedure.

3. This order becomes effective from the date of its official publication.

4. To impose control of execution of this order on the deputy minister of internal affairs of Ukraine Goncharov S. M.

Temporary procedure for registration and issue of the passport of the citizen of Ukraine

I. General provisions

1. This Temporary procedure developed according to the paragraph to the fifth Item 3 of the resolution of the Cabinet of Ministers of Ukraine of March 25, 2015 No. 302 "About approval of sample of the form, technical description and the Procedure for registration, issue, exchange, transfer, withdrawal, return to the state, recognitions invalid and destructions of the passport of the citizen of Ukraine", to the resolution of the Cabinet of Ministers of Ukraine of April 3, 2019 No. 398 "About modification of Item 3 of the resolution of the Cabinet of Ministers of Ukraine of March 25, 2015 No. 302", to the Regulations on the passport of the citizen of Ukraine approved by the Resolution of the Verkhovna Rada of Ukraine of June 26, 1992 No. 2503-XII (in edition of the Resolution of the Verkhovna Rada of Ukraine of February 23, 2007 No. 719-V), determines procedure for submission of documents, their considerations and decision making about registration and issue of the passport of the citizen of Ukraine of sample of 1994 (further - the passport) to person on whom the judgment is made, taken legal effect, about obligations of GMS to draw up and issue the passport of the citizen of Ukraine of sample of 1994 (further - the judgment), certified in the procedure established by the legislation.

2. The passport is drawn up with use of the form of the passport of the citizen of Ukraine of the sample approved by the resolution of the Cabinet of Ministers of Ukraine of June 4, 1994 No. 353 "About approval of sample of the form of the passport of the citizen of Ukraine".

3. Registration and issue of the passport are performed by territorial subdivisions of the Public migratory service of Ukraine (further - the GMS territorial subdivisions):

1) to person who reached 16-year age, - based on the statement for issue of the passport of the citizen of Ukraine (further - the statement) on the sample given in appendix 1 to this Temporary procedure given them personally;

2) to person who reached 16-year age and recognized as court it is limited capable or incapacitated, - based on the statement of one of parents (adoptive parents), guardians, custodians or other legal representatives (further - legal representatives).

The statement is consent to processing of personal data according to the Law of Ukraine "About personal data protection". During filling of the statement with person or his legal representative (further - the applicant) the employee of the GMS territorial subdivision informs the applicant on requirements of the Law of Ukraine "About personal data protection".

4. Registration and issue of the passport are performed within 30 calendar days from the date of giving by person in the GMS territorial subdivision of the statement and documents for registration and issue of the passport.

5. The statement and documents for registration and issue of the passport (including for gluing in of the photo) are represented by the applicant to the GMS territorial subdivision in the place of the address.

Homeless person submits the application and documents to the GMS territorial subdivision on territorial arrangement of the centers of accounting where they stay on the registry, or organizations of social protection where social services are provided to them. 

6. The documents issued by competent authorities of foreign state which are filed for execution of the passport, are certified of the procedure established by the legislation if other is not provided by international treaties which consent to be bound is provided by the Verkhovna Rada of Ukraine and move with the translation into Ukrainian which fidelity makes sure notarially.

7. The application form is filled in personally by the applicant by hand, legible handwriting, without reducings and corrections and own signature provision of consent to processing of personal data according to the Law of Ukraine "About personal data protection" validates the data entered in it, and also. 8. After filling with the applicant of the statement the employee of the GMS territorial subdivision checks correctness of filling of the statement and its compliance to the provided documents about what makes the corresponding entry in column 9 of the statement.

9. In column 9 of the statement the employee of the GMS territorial subdivision specifies details of the documents which are the basis for execution of the passport (the certificate about the birth/document confirming circumstances based on which the passport is subject to exchange / person who is lost/kidnapped passports / certificates of registration by the citizen of Ukraine and the judgment). In case of submission of the certificate of registration of person by the citizen of Ukraine in the column "Office Marks" number and date of the decision on acquisition-faced citizenship of Ukraine and the country of the previous nationality are specified.
