About approval of the Application process on production of technical means of rehabilitation by the individual order
According to Item 42 of the Procedure for ensuring rehabilitation of persons with technical and other means with disability, children with disability and other separate categories of the population and payment of monetary compensation of cost for independently acquired technical means of rehabilitation, the Cabinet of Ministers of Ukraine approved by the resolution of April 5, 2012 No. 321 (with changes), PRIKAZYVAYU:
1. Approve the Application process on production of technical means of rehabilitation by the individual order which is attached.
2. Declare invalid the order of the Ministry of social policy of Ukraine of August 1, 2014 No. 518 "About approval of the Application process on production of technical means of rehabilitation by the individual order", registered in the Ministry of Justice of Ukraine on August 26, 2014 for No. 1028/25805.
3. (Vishnevskaya N. V.) to provide to department of implementation of the state social programs in the procedure established by the legislation submission of this order on state registration in the Ministry of Justice of Ukraine.
4. This order becomes effective from the date of its official publication.
5. To impose control of execution of this order on the deputy minister Popova L. V.

1. This Procedure determines procedure of application for production of technical means of rehabilitation for the individual order, registration and issue of technical means of rehabilitation for the individual order (except prosthetic and orthopedic products, including orthopedic footwear).
2. Medical indications to ensuring rehabilitation with technical means determine by the individual order for:
persons with disability - medico-social commissions of experts (further - MSEK) by execution of the corresponding conclusion and the individual program of rehabilitation of the disabled person (further - the International Party of Russia);
children with disability - the medical and advisory commissions of treatment and prevention facilities (further - VKK) by execution of the corresponding conclusion;
persons with disability and children with disability who reached six-year age (for providing with chairs wheel with the electric drive for movement indoors); children with disability who reached fourteen-year age (for providing with chairs wheel with the electric drive for movement out of the room), - regional, central city in Kiev and Sevastopol, republican MSEK in the Autonomous Republic of Crimea with execution of the corresponding medical certificate;
