According to Item 8 of the Regulations on the Ministry of social policy of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of June 17, 2015 to No. 423 and Item 17 of the Payment procedure of monetary compensations on gasoline, repair and maintenance of cars and on transport servicing, approved by the resolution of the Cabinet of Ministers of Ukraine of February 14, 2007 No. 228, I ORDER:

1. Approve such which are applied:

1) reporting No. 8 form "The report on payment of monetary compensations on gasoline, repair and maintenance of cars and on transport servicing" (further - No. form 8);

2) the Instruction for filling of form of the reporting No. 8 "The report on payment of monetary compensations on gasoline, repair and maintenance of cars and on transport servicing".

2. (Vishnevskaya N. V.) to provide to department of implementation of the state social programs:

1) submission of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine;

2) representation of statistics of Ukraine to Public service of the summarized information in form No. 8 in Ukraine in general and on administrative and territorial units annually till March 15 of the year following reporting.

3. To executive body of the Autonomous Republic of Crimea concerning social protection of the population, to structural divisions concerning social protection of the population of the regional, Kiev and Sevastopol city public administrations to provide:

1) collection, processing and generalization of information on form No. 8;

2) submission of the generalized report on form No. 8 to the Ministry of social policy at the scheduled time.

4. Declare invalid the order of the Ministry of social policy of Ukraine of August 16, 2013 No. 512 "About approval of form of the reporting No. 8 "The report on payment of monetary compensations on gasoline, repair and maintenance of cars and on transport servicing" and Instructions for its filling", registered in the Ministry of Justice of Ukraine on September 3, 2013 for No. 1515/24047 (with changes).

5. This order becomes effective from the date of its official publication.

6. To impose control of execution of this order on the deputy minister Popova L. V.

The instruction for filling of form of the reporting No. 8 "The report on payment of monetary compensations on gasoline, repair and maintenance of cars and on transport servicing"

I. General provisions

1. The reporting No. 8 form "The report on payment of monetary compensations on gasoline, repair and maintenance of cars and on transport servicing" (further - the Report) contains indicators of assignment and payment of monetary compensations to gasoline, repair and maintenance of cars and on transport servicing to persons with disability, to legal representatives of incapacitated persons with disability, children with disability who according to the legislation have the right to providing with cars.

2. The report is constituted twice a year: for the I half-year - till July 10 and for the whole year - till January 10 of the year following for reporting, structural divisions concerning social protection of the population of district, district public administrations, actuators in the cities of Kiev and Sevastopol city, district in the cities (except the cities of Kiev and Sevastopol) councils on check-in location (stay) of persons with disability, legal representatives of incapacitated persons with disability, children with disability who have the right to payment of monetary compensations to gasoline, repair and maintenance of cars and to transport servicing and it is represented to executive body of the Autonomous Republic of Crimea concerning social protection of the population, to structural divisions concerning social protection of the population of the regional, Kiev and Sevastopol city public administrations.

3. Executive body of the Autonomous Republic of Crimea concerning social protection of the population, structural divisions concerning social protection of the population of the regional, Kiev and Sevastopol city public administrations twice a year: for the I half-year - till July 20 and for the whole year - till January 20 of the year following the accounting period submit the Report to the Ministry of social policy of Ukraine.

4. The ministry of social policy of Ukraine till March 15 of the year following reporting submits the Report on Ukraine and administrative and territorial units to Public service of statistics of Ukraine.

5. When filling the Report those units of measurement which are specified in forms are used.

6. The report shall be completed is legible and is signed by the head and persons responsible for its filling, with indication of phone numbers, faxes and e-mail addresses.
