According to the Laws of Ukraine "About the government public assistance to persons with disability since the childhood and to children with disability" and "About the government public assistance to persons who do not have pension entitlements and to persons with disability", to Item 8 of the Regulations on the Ministry of social policy of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of June 17, 2015 No. 423 (with changes), PRIKAZYVAYU:

Permit to use till January 1, 2020 for issue to persons who receive the government public assistance according to the Laws of Ukraine "About the government public assistance to persons with disability since the childhood and to children with disability" and "About the government public assistance to persons who do not have pension entitlements and to persons with disability", earlier made forms of certificates on the sample approved by the order of the Ministry of Labour and Social Policy of Ukraine of November 16, 2007 No. 612 "About approval of the Accounting treatment, storage, registration and certification to persons who receive the government public assistance according to the Law of Ukraine "About the government public assistance to disabled people since the childhood and to handicapped children", registered in the Ministry of Justice of Ukraine on December 5, 2007 for No. 1349/14616 (with changes).
3. Determine that the certificates issued before entry into force of this order and the certificates issued according to Item 2 of this order are valid before the termination of their effective period or before their replacement in accordance with the established procedure for new.
4. Declare invalid the order of the Ministry of Labour and Social Policy of Ukraine of November 16, 2007 No. 612 "About approval of the Accounting treatment, storage, registration and certification to persons who receive the government public assistance, according to the Law of Ukraine "About the government public assistance to disabled people since the childhood and to handicapped children", registered in the Ministry of Justice of Ukraine on December 05, 2007 for No. 1349/14616 (with changes).
5. To provide to management of family and social support of the population (V. Muzychenko) submission in accordance with the established procedure of this order on state registration in the Ministry of Justice of Ukraine.
6. To impose control of execution of this order on the deputy minister L. Popova.
7. This order becomes effective from the date of its official publication.
