According to Item 9 parts one of article 7 of the Law of Ukraine "About licensing of types of economic activity", Article parts fifteen 4, to the paragraph to the eighth part two of article 5 of the Law of Ukraine "About the basic principles of the state supervision (control) in the field of economic activity", parts five of article 41-1 of the Law of Ukraine "On regulation of town-planning activities", the Technique of development of the unified forms of statements which are drawn up by results of holding planned (unplanned) actions of the state supervision (control), Ukraine approved by the resolution of the Cabinet of Ministers of May 10, 2018 No. 342 (with changes), PRIKAZYVAYU:

1. Approve the unified form of the statement which is drawn up by results of holding planned (unplanned) action of the state supervision (control) for observance by the subject of managing of requirements of the legislation in the field of implementation of economic activity on construction of facilities which on class of effects (responsibility) belong to objects with average and considerable effects which is applied.

2. Declare invalid the order of the Ministry of Regional Development, Construction and Housing of Ukraine of December 21, 2016 No. 331 "About approval of the unified form of the statement which is drawn up by results of conducting scheduled (unplanned) inspection behind observance by the subject of managing of licensed conditions of implementation of economic activity on construction of facilities which on class of effects (responsibility) belong to objects with average and considerable effects", registered in the Ministry of Justice of Ukraine on January 18, 2017 for No. 73/29941 (with changes).

3. To department of town planning, architecture and planning of the territories (Belous S. Ya.), to Legal department (Chepelyuk A. V.) together with the State architectural building control department of Ukraine (Filonchuk V. V.) to submit this order in accordance with the established procedure on state registration to the Ministry of Justice of Ukraine.

4. This order becomes effective from the date of its official publication.

5. To put control over the implementation of this order on the deputy minister Partskhaladze L. R.
