1. Approve the enclosed list of the documents testimonial of observance by the owner of audiovisual service of requirements of part 7 of article 10.5 of the Federal law "About Information, Information Technologies and on Information Protection".

2. Determine that:

the documents and data specified in Items 2 - the 7th list, are represented concerning all persons listed in part 7 of article 10.5 of the Federal law "About Information, Information Technologies and on Information Protection";

the documents specified in the list, constituted in foreign language shall be legalized in accordance with the established procedure, translated into Russian and transfer shall be notarially certified;

the documents testimonial of observance by the owner of audiovisual service of requirements of part 7 of article 10.5 of the Federal law "About Information, Information Technologies and on Information Protection" are represented to Federal Service for Supervision in the Sphere of Telecom, Information Technologies and Mass Communications within 2 months from the date of receipt by the owner of audiovisual service of the notification on inclusion of its information resource in the register of audiovisual services, and in case of change of the information about the owner of audiovisual service containing in the register of audiovisual services - within 2 months from the date of such changes;

the documents submitted in electronic form are signed by the strengthened qualified digital signature of the owner of audiovisual service;

paper documents are submitted to Federal Service for Supervision in the Sphere of Telecom, Information Technologies and Mass Communications in one copy;

the package of documents which is included the documents specified in the list, constituted in foreign language, legalized in accordance with the established procedure and translated to Russian is represented on paper;

the documents specified in the list, earlier provided by the applicant in case of the appeal to the government commission operating according to part 9 of article 10.5 of the Federal law "About Information, Information Technologies and on Information Protection" repeatedly are not represented.
