About approval of the Procedure for issue of the medical certificate for receipt of permission (license) for object of authorization system

According to resolutions of the Cabinet of Ministers of Ukraine of 12.10.92 N576 "About Approval of the Regulations on Authorization System" and of 07.09.93 N706 "About Procedure for Sale, Acquisition, Registration, Accounting and Application of the Special Means of Self-defense Loaded with Substances Tear and Irritant Action", and also for the purpose of ordering of issue of the medical certificate for receipt of permission (license) for object of authorization system

I ORDER:

1. Approve the Procedure for issue of the medical certificate for receipt of permission (license) for object of authorization system it (is applied).

2. To provide to the Minister of Health of the Autonomous Republic of Crimea, chiefs of managements of health care of the regional, Kiev and Sevastopol city state administrations observance of procedure for issue of the medical certificate for receipt of permission (license) for object of authorization system according to requirements of this order.

Item 3 is excluded according to the Order of the Ministry of Health of Ukraine of 21.02.2013 No. 149, with respect thereto item 4 to consider Item 3.

Procedure for issue of the medical certificate for receipt of permission (license) for object of authorization system

1. The medical certificate for receipt of permission (license) for object of authorization system (f. N127 / about (accounting), appendix 1, further - the medical certificate) is the single document which is issued to the citizen by the medical and advisory commission after performing medical examination for the purpose of permission of question of availability or lack of medical contraindications for receipt of permission (license) for object of authorization system according to resolutions of the Cabinet of Ministers of Ukraine of October 12, 1992 N576 "About Approval of the Regulations on Authorization System" and of September 7, 1993 N706 "About Procedure for Sale, Acquisition, Registration, Accounting and Application of the Special Means of Self-defense Loaded with Substances Tear and Irritant Action".

 Item 2 is excluded according to the Order of the Ministry of Health of Ukraine of 21.02.2013 No. 149, with respect thereto Items 3-14 to consider respectively Items 2-13.

2. Destruction of the damaged forms of medical certificates is carried out in the presence of the commission which is formed by the head of treatment-and-prophylactic institution, and drawn up by the relevant act.

3. Physical examinations for the purpose of issue of the medical certificate are conducted for customer account.

4. The issue of issue of the medical certificate is resolved by the medical and advisory commission which part are: commission chairman (therapist), doctor-neuropathologist, otolaryngologist, ophthalmologist. The list of laboratory and functional inspections includes obligatory amount (general blood test and urine, blood test on sugar, the ECG, inspection of sharpness and field of vision), and also individual additional inspections which are appointed by doctors - members of the medical and advisory commission. In case of permission of question of issue of the medical certificate the results of obligatory routine narcological inspection of the citizen yielded relatively stay of the citizen on psychiatric accounting are surely considered.

5. In case of permission of question of receipt of the medical certificate the citizen shall certify the personality (to provide the passport), to submit the certificate on passing of obligatory routine narcological inspection, the reference concerning stay of the citizen on psychiatric accounting, 2 photos of 3 x 4 cm in size, the document confirming fee connected with issue of the medical certificate.

The medical certificate without any of the documents stated above is not issued.

6. In case of requirement the commission has the right to involvement of health workers whose participation is not provided by the list of the specialists who are part of the commission.

7. Data of physical examinations are entered by doctors of the medical and advisory commission in the Medical card of the outpatient (f. N025 / about). In this card the certificate on passing of obligatory routine narcological inspection, the reference concerning stay of the citizen on psychiatric accounting are surely pasted.

In the presence at the citizen of diseases according to the List of medical contraindications for receipt of permission (license) for object of authorization system (appendix 2) the medical and advisory commission takes out the conclusion concerning availability of medical contraindications for receipt of permission (license) for specific object of authorization system. In case of absence at the citizen of diseases according to the specified List the medical and advisory commission takes out the conclusion concerning lack of medical contraindications for receipt of permission (license) for specific object of authorization system.

8. After performing medical examination the medical and advisory commission issues to the citizen the medical certificate with indication of availability or absence at it medical contraindications for receipt of permission (license) for specific object of authorization system. The maximum effective period of the medical certificate is determined, proceeding from data of medical examination, and also effective period of the certificate on passing of obligatory routine narcological inspection provided by the citizen (f. N140-0 (accounting)), but cannot exceed three years.

The medical certificate signed by members and the chairman of the medical and advisory commission is certified by seal treatment-and-prophylactic institution.

9. The citizen has the right to receive the duplicate of the issued medical certificate throughout the complete term of its action. At the same time additional physical examinations are not conducted, the citizen pays only the cost of the form of the medical certificate.

10. The medical and advisory commission does not perform medical examination:

in the presence at the inspected person of acute disease or exacerbation of chronic noninfectious disease;

in the presence at the inspected person of infectious disease in the infectious period;

in case of stay of the inspected person in state of intoxication of any origin.
