1. Approve the enclosed Procedure and requirements to execution of the document on the taken place event on transport and its circumstances for receipt of insurance indemnity on obligatory civil liability insurance of carrier for damnification of life, to health, property of passengers (further - the Procedure).

2. Provisions of this Procedure do not extend to cases of damnification of life, to health, property of passengers on sea transport, and also property torts of the passenger on inland water transport.

Procedure and requirements to execution of the document on the taken place event on transport and its circumstances for receipt of insurance indemnity on obligatory civil liability insurance of carrier for damnification of life, to health, property of passengers

1. This Procedure determines rules of execution of the document on the taken place event on transport and its circumstances for receipt of insurance indemnity by the agreement of obligatory civil liability insurance of carrier for damnification of life, to health, property of passengers (further - the agreement of compulsory insurance) and confirmations of the fact of loss occurrence under the agreement of compulsory insurance, and also establishes requirements to contents of this document.

2. The terms and determinations used in this Procedure correspond to terms and determinations, the established Federal Law of June 14, 2012 No. 67-FZ "About obligatory civil liability insurance of carrier for damnification of life, to health, property of passengers and about procedure for compensation of such harm done in case of public conveyances by the subway" (The Russian Federation Code, 2012, No. 25, Art. 3257; 2013, No. 30, Art. 4084, No. 49, Art. 6333; 2014, No. 45, Art. 6154; 2016, No. 22, Art. 3094, No. 26, Art. 3891; 2017, No. 31, Art. 4826).

3. The document on the taken place event on transport and its circumstances (further - the document on the taken place event) shall be constituted by the carrier or person authorized by carrier concerning each event of event as a result of which harm was in transit done to life, health, property of the passenger.

4. In case of damnification to baggage of the passenger on air <1> and automobile <2> transport the document on the taken place event hereinafter is referred to as as the commercial act.

2) information about carrier and type of transport, including:

identification taxpayer number (ITN);

full name of carrier or surname, name, middle name (in the presence) individual entrepreneur;

mode of transport;

type of transport;

number (name) of route (run) - in the presence;
