According to the Order of interaction of subjects of social maintenance of families (persons) which are in difficult vital circumstances, to No. approved by the resolution of the Cabinet of Ministers of Ukraine of November 21, 2013 895, to the Procedure for identification of families (persons) which are in difficult vital circumstances, provisions by it of social services and implementation of social maintenance of such families (persons), to No. approved by the resolution of the Cabinet of Ministers of Ukraine of November 21, 2013 896, and for the purpose of enhancement of system of accounting of work with families/persons who are in difficult vital circumstances, PRIKAZYVAYU:

1. Approve the enclosed forms of accounting of social work with families/persons which are in difficult vital circumstances:

Message/information on family/person, being in difficult vital circumstances;

Appraisal report of needs of family/person;

Report on results of social escort of family/person;

Social card of family/person;

Plan of social escort of family/person;

The direction of family/person to other subject for provision of social services.

2. Declare invalid the order of the Ministry of social policy of July 9, 2014 No. 450 "About approval of forms of accounting of social services to families (persons) which are in difficult vital circumstances", registered in the Ministry of Justice of Ukraine on September 04, 2014 for No. 1076/25853.

3. To management of the integrated social services (Pinchuk I. M.) provide submission in accordance with the established procedure of this order on state registration in the Ministry of Justice of Ukraine.

4. To impose control of execution of this order on the deputy minister concerning the European integration Churkina O. I.

5. This order becomes effective from the date of its official publication.
