About approval of regulations of the state services in the field of health care

1. Approve regulations of the state services:

1) "Issue of information on the transferred amounts of assignments and (or) fees on compulsory social medical insurance" according to appendix 1 to this order;

2) "Issue of certificates of participation in system of compulsory social medical insurance as the consumer of medical services" according to appendix 2 to this order.

2. To provide to department of coordination of compulsory social medical insurance of the Ministry of Health of the Republic of Kazakhstan:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) within ten calendar days from the date of state registration of this order the direction it the copy in paper and electronic type in the Kazakh and Russian languages in the Republican state company on the right of economic maintaining "The republican center of legal information" of the Ministry of Justice of the Republic of Kazakhstan for official publication and inclusion in Reference control bank of regulatory legal acts of the Republic of Kazakhstan;

3) within ten calendar days after state registration of this order the direction it the copy on official publication in periodic printing editions;

4) placement of this order on Internet resource of the Ministry of Health of the Republic of Kazakhstan;

5) within ten working days after day of state registration of this order in the Ministry of Justice of the Republic of Kazakhstan submission to Department of legal service of the Ministry of Health of the Republic of Kazakhstan of data on execution of the actions provided by subitems 1), 2), 3) and 4) of this Item.

3. To impose control of execution of this order on the vice-Minister of Health of the Republic of Kazakhstan Aktayeva L. M.

4. This order becomes effective after ten calendar days after day of its first official publication.

Regulations of the state service "Issue of Information on the Transferred Amounts of Assignments and (or) Fees on Compulsory Social Medical Insurance"

Chapter 1. General provisions

1. The regulations of the state service "Issue of Information on the Transferred Amounts of Assignments and (or) Fees on Compulsory Social Medical Insurance" (further - Regulations) are developed according to the Standard of the state service "Issue of Information on the Transferred Amounts of Assignments and (or) Fees on Compulsory Social Medical Insurance" (further - the Standard) approved by the order of the Minister of health and social development of the Republic of Kazakhstan of April 27, 2015 No. 272 (it is registered in the Register of state registration of regulatory legal acts at No. 11304).

2. The state service appears non-commercial joint-stock company "The Government for Citizens state corporation (further - the service provider).

Reception of applications and issue of results of rendering the state service is performed through the service provider.

3. Form of rendering the state service: paper.

4. Issue of information on the transferred amounts of assignments and (or) fees on compulsory social medical insurance or the motivated answer about refusal in rendering the state service in the bases, stipulated in Item 10 Standards is result of rendering the state service.

Form of provision of result of rendering the state service: paper.

Chapter 2. The description of operations procedure of structural divisions (workers) of the service provider in the course of rendering the state service

5. The basis for the procedure (action) for rendering the state service is provision by uslugopoluchatel of the documents specified in Item 9 of the Standard.

6. Contents of each procedure (action) which is part of process of rendering the state service:

The worker of the service provider accepts 1) and checks completeness of the submitted documents - 30 (thirty) minutes;

2) the worker of the service provider registers statements from the moment of receipt from uslugopoluchatel of the statement - 1 (one) hour;

3) the worker of the service provider from the date of obtaining considers applications it and draws up result of rendering the state service - 8 (eight) working days;

4) the worker of the service provider makes issue of result to uslugopoluchatel since the moment receipt of result - 1 (one) working day.

Chapter 3. The description of procedure for use of information systems in the course of rendering the state service

7. The description of procedure for the address and the sequence of the procedure through the service provider are given in appendix to these Regulations:

1) process 1 - input by the employee in the automated workplace of information system (further - IS) login and the password (authorization process) for rendering service;

2) process 2 - the choice by the employee of the service specified in these Regulations, conclusion to the request form screen for rendering service and input by the employee of data of uslugopoluchatel and also data by proxy the representative of uslugopoluchatel (in case of notarially certified power of attorney, in case of other certificate of the power of attorney data of the power of attorney are not filled);

3) process 3 - sending an inquiry through lock of the electronic government in the State database of physical persons (further - GBD FL) about data of uslugopoluchatel, and also in the Single notarial information system (further - ENIS) - about data of the power of attorney of the representative of uslugopoluchatel;

4) condition 1 - verification of presence of data of uslugopoluchatel in GBD FL this to the power of attorney in ENIS;
