About approval of the unified form of the statement which is drawn up by results of holding planned (unplanned) action of the state supervision (control) for observance by the subject of managing of requirements of the legislation in the field of veterinary medicine

According to the paragraph to the eighth part two of article 5 of the Law of Ukraine "About the basic principles of the state supervision (control) in the field of economic activity", to Item of 1 part one of article 6 of the Law of Ukraine "About the state control of compliance with law about foodstuff, sterns, by-products of animal origin, health and wellbeing of animals", to Item 2 parts seven of article 7 of the Law of Ukraine "About veterinary medicine", order to the Technique of development of the unified forms of the statements which are drawn up by results of holding planned (unplanned) actions of the state supervision (control), Ukraine approved by the resolution of the Cabinet of Ministers of August 28, 2013 No. 752,:

1. Approve the unified form of the statement which is drawn up by results of holding planned (unplanned) action of the state supervision (control) for observance by the subject of managing of requirements of the legislation in the field of veterinary medicine which is applied.

2. Declare invalid the order of the Ministry of Agricultural Policy and Food of Ukraine of October 12, 2011 No. 538 "About approval of lists of questions for implementation of planned actions of the state supervision (control) of observance by subjects of managing of the legislation in the field of veterinary medicine and the unified inspection statement form", the baking plate No. 1256/19994 registered in the Ministry of Justice of Ukraine on November 2, 2011.

3. To provide to department of agrarian policy and agricultural industry in accordance with the established procedure providing this order on the state registration in the Ministry of Justice of Ukraine.

4. This order becomes effective from the date of its official publication.

5. To impose control of execution of this order on the First Deputy Minister M. Martynyuk.
