In pursuance of provisions of the h. (5) Art. 92 and h. (2) Art. 133 of the Tax code No. 1163-XIII of April 24, 1997 (repeated publication in Monitorul Oficial al Republicii Moldova, 2007, special release of February 8 2007), with subsequent changes and amendments, I ORDER:

1. Approve:

- the standard form "Information on the Salary and on Other Payments Performed by the Employer for benefit of Workers and also on the Payments Made to Residents from the Sources of the Income Other than the Salary and on the Income Tax Withheld from These Payments" (the IALS14) form, according to appendix No. 1, and the Procedure for its filling, according to appendix No. 2;

- the standard form "Information on the Income Tax Withheld from the Sources of the Income Other than the Salary Paid to Nonresidents" (the INR14) form, according to appendix No. 3, and the Procedure for its filling, according to appendix No. 4;

- the standard form "Information on the Income Added and Paid for benefit of Physical (Legal) Person and on the Income Tax Withheld from This Income", according to appendix No. 5.
