1. Approve enclosed:

notification form of the specific subsidies provided by state member of the Eurasian Economic Union approved with the Eurasian economic commission;

Procedure for filling of form of the notification on the specific subsidies provided by state member of the Eurasian Economic Union approved with the Eurasian economic commission.

2. This Decision becomes effective from the effective date Agreements on procedure for voluntary approval by state members of the Eurasian Economic Union with the Eurasian economic commission of specific subsidies concerning manufactured goods and conducting by the Eurasian economic commission the trials connected with provision by state members of the Eurasian Economic Union of specific subsidies of May 26, 2017, but not earlier than after 10 calendar days from the date of official publication of this Decision.

Procedure for filling of form of the notification on the specific subsidies provided by state member of the Eurasian Economic Union approved with the Eurasian economic commission

I. General provisions

1. This Procedure is developed according to Item 7 of article 12 of the Agreement on procedure for voluntary approval by state members of the Eurasian Economic Union with the Euroasian economic commission of specific subsidies concerning manufactured goods and conducting by the Euroasian economic commission the trials connected with provision by state members of the Eurasian Economic Union of specific subsidies of May 26, 2017 (further - the Agreement) and determines rules of filling of form of the notification on the specific subsidies provided by state member of the Eurasian Economic Union approved with the Euroasian economic commission approved by the Decision of Council of the Euroasian economic commission of December 20, 2017 № 95 (further - notification form). 

2. The concepts used in this Procedure are applied in the values established by the Agreement and the Protocol on single rules of provision of industrial subsidies (appendix №28 to the Agreement on the Eurasian Economic Union of May 29, 2014).

3. The form of the notification is filled with authorized bodies of state members of the Eurasian Economic Union (further - state member) with use of technical means of information input separately on each provided specific subsidy approved with the Eurasian economic commission (further - the Commission).

4. If in the form of the notification there is not enough place for specifying of necessary data, such data are drawn up by the stand-alone program (in a free form). At the same time in the corresponding Item of form of the notification record "according to appendix №" is made. If several appendices are drawn up, they are numbered consistently in ascending order.

II. Rules of filling of form of the notification

5. The form of the notification is filled in the following procedure.

6. In Item 1 the name, acceptance date and number of the decision of Board of the Commission on admissibility of specific subsidy are specified.

7. In Item 2 the criteria of admissibility of specific subsidy determined according to Item 1 of article 11 of the Agreement and (or) Eurasian intergovernmental council according to Item 3 of article 11 of the Agreement based on which the specific subsidy is approved by the Commission are specified.

8. In Item 3 the name, acceptance date and number of the act according to which the specific subsidy, the short description of its content, the name, acceptance date and number of the act adopted in implementation of the act (in the presence) and also the name and acceptance date of the program of subsidizing is provided are specified.

9. In item 4 accounting year for which data on specific subsidy are represented is specified.

10. In Item 5 the form of financial assistance as a result of which are created is specified benefits, or other form of support of the income or the prices which affects (directly or indirectly) reducing import of industrial goods from the territory of any state member or increase in export of industrial goods at the territory of any state member therefore the benefit is provided (are provided).

11. In Item 6 the purposes and tasks of provision of specific subsidy determined in the acts specified in Item 3 forms of the notification are specified.
