Some questions of procedure for execution of certificates on accreditation (their duplicates)

According to the Laws of Ukraine "About education", "About the higher education", the resolution of the Cabinet of Ministers of Ukraine of April 29, 2015 to No. 266 "About approval of the list of industries of knowledge and specialties on which training of job seekers of the higher education is performed" of August 9, 2001 No. 978 "About approval of the Regulations on accreditation of higher educational institutions and specialties in higher educational institutions and the highest professional schools", the order of the Ministry of Education and Science of Ukraine of November 6, 2015 No. 1151 "About features of introduction of the list of industries of knowledge and specialties on which training of job seekers of the higher education approved by the resolution of the Cabinet of Ministers of Ukraine of April 29, 2015 No. 266" registered in the Ministry of Justice of Ukraine on November 25, 2015 at No. 1460/27905, is performed I order:

1. Approve sample of the certificate on accreditation which is applied.

2. Make changes to the Procedure for registration, issue, storage and accounting of certificates on accreditation (their duplicates) approved by the order of the Ministry of Education and Science of Ukraine of October 15, 2001 No. 680, registered on December 6, 2001 at No. 1008/6199, having been reworded as follows it which is applied the Ministry of Justice of Ukraine.

3. To department of certification of personnel of the top skills and licensing (Shevtsov A.) provide state registration of this order in the Ministry of Justice of Ukraine and its placement on the official website of the Ministry of Education and Science of Ukraine.

4. To management of administrative and organizational support (Erko I. A.) make mark for archive.

5. To impose control of execution of this order on the First Deputy Minister Kovtunts V. V.

6. This order becomes effective from the date of its official publication.

Procedure for registration, issue, storage and accounting of certificates on accreditation (their duplicates)

I. General provisions

1. This Procedure developed according to Item 2 of the resolution of the Cabinet of Ministers of Ukraine of August 9, 2001 No. 978 "About approval of the Regulations on accreditation of higher educational institutions and specialties in higher educational institutions and the highest professional schools" determines procedure for registration, issue, storage and accounting of certificates on accreditation (their duplicates).

2. The certificate on accreditation (further - the certificate) - the document which confirms compliance of level of training of specialists in certain specialty (the corresponding direction) on certain level of the higher education to the state requirements or compliance of the educational program of institution of higher education for the corresponding specialty and level of the higher education to the standard of the higher education and grants the right to issue of the diploma of the state sample in this specialty of the appropriate educational program.

II. Registration and issue of certificates on accreditation (their duplicates)

1. MON draws up and issues to institution of higher education the certificate within twenty working days from receipt date of its statement for issue of the certificate based on the positive decision of the accreditation commission approved according to the procedure, determined in the Regulations on accreditation of higher educational institutions and specialties in higher educational institutions and the highest professional schools approved by the resolution of the Cabinet of Ministers of Ukraine of August 9, 2001 No. 978.

2. In the certificate specify:

the name of the body issuing the certificate;

series and certificate number;

accredited specialty of certain knowledge domain with appropriate level of the higher education or accredited educational (educational and professional, educational and scientific or educational and creative) the program for the corresponding specialty and level of the higher education;

the name, identification code, the location of institution of higher education - the legal entity (in case of accreditation in the isolated structural division - the name of such division), scientific institution;

certificate effective period;

basis, acceptance date and number of the decision on accreditation;

position, surname and initials of person which signed the certificate;

date of issue of the certificate.

The certificate is signed by the Minister of Education and Science of Ukraine and assures official stamp of MON.

3. Series of the certificate consists of two line items:

1) the first line item of series determines type of the institution of higher education accredited as certain knowledge domain or the educational program:

letter U - university;

letter A - academy;

letter I - institute;

letter K - college;

2) the second line item of series determines pattern of ownership on which the institution of higher education is founded:

letter G - the state pattern of ownership;

letter K - utility pattern of ownership;

letter Ch - private pattern of ownership.

4. The certificate number contains eight figures from which the first two specify administrative and territorial unit of Ukraine in which there is institution of higher education:
