According to Item 64.5 of Article 64 of Chapter 6 of the Section II of the Tax code of Ukraine and Regulations on the Ministry of Finance of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of August 20, 2014 No. 375, I order:

1. Approve form of the Act of establishment of signs of conducting economic activity by the nonresident in the territory of Ukraine without capture on tax accounting of the nonresident, in particular, of implementation by the resident of activities for the benefit of the nonresident through separate divisions, including that has signs of permanent mission which is applied.

2. Declare invalid the order of the Ministry of the income and charges of Ukraine of November 22, 2013 No. 696 "About approval of form of the Act of factual determination of conducting economic activity by the nonresident through permanent mission in the territory of Ukraine without statement on tax accounting", registered in the Ministry of Justice of Ukraine on December 6, 2013 at No. 2077/24609.

3. In accordance with the established procedure to provide to department of tax policy of the Ministry of Finance of Ukraine:

provision of this order on state registration in the Ministry of Justice of Ukraine;

promulgation of this order.

4. This order becomes effective from the date of its official publication.

5. I reserve control of execution of this order and I assign to the acting as the Chairman of the Public fiscal service of Ukraine M. V Is sold.
